How we evaluate panagglutinating sera

Abstract: P anagglutinating sera is one of the most challenging dilemmas of the antibody identification process. It occurs when patient sera react with all red blood cells (RBCs) tested, that is, with both screening and identification panel cells used in first approach. In this situation systematic workup is necessary to reduce the risk of error and optimize sample use. Two main problems must be resolved. The first is to determine whether panagglutination is due to an auto-or alloantibody against a high-frequency antige… Show more

“…The indication of transfusing patients infected with HIV/AIDS does not differ substantially compared with HIV-negative [25]. However, the main immunohematological problem is the positivity of pretransfusional crossmatch test (Figure 3) [10,26]. Prior to any attempt at resolution, always consider the patient's antecedents (transfusion, pregnancy, drugs, etc.)…”

“…These may be because of an antibody to a high prevalence antigen or antibodies to multiple common specificities. An article from France showed a problem solving strategy in such cases [48]. Often, in complex cases, phenotyping or genotyping of the patient’s red cells is used along with antibody identification techniques such as chemical treatments of reagent red cells and adsorption studies to separate the specificities when multiple specificities are present.…”

“…The recent article by Chiaroni and colleagues 1 provided an algorithm for the serologic investigation of panagglutinating sera that is performed in their laboratory in Marseille, France. I feel that their algorithm is incomplete and suboptimal for a rapid identification of multiple and/or high‐frequency alloantibody that may be present in certain patients.…”
