Poor mental health in doctors appears to be a global phenomenon, but there are limited data on changes in doctors' psychological symptoms over time in mainland China. Through a detailed meta-analysis of cross-sectional studies, our aim was to examine the prevalence of psychological symptoms in Chinese physicians as measured with the Symptom Checklist 90-R (SCL-90-R) and to explore the factors associated with doctors' mental health. A comprehensive search was performed in major English and Chinese databases. Thirty studies involving a total of 6,099 subjects were included in the meta-analysis. The pooled estimates of psychological symptoms including somatization, obsession-compulsion, interpersonal-sensitivity, depression, anxiety, hostility, phobic anxiety, and paranoid ideation among doctors were significantly higher than those in the general population. Only psychoticism was similar in prevalence to Chinese population norms. The prevalence increased with the study year but decreased with physicians' increasing age. Doctors from central and western China experienced more mental health symptoms than those from eastern China. Psychiatrists scored significantly less favorably than other doctors on most subscales of the SCL-90-R. Doctors' mental health may be associated with age discrepancy, quantitative workload, effort-reward ratio, doctor-patient relationships, professional identity, and individual traits. To minimize the risk of poor mental health in doctors, screening and professional intervention services should be provided at early career stages to raise physicians' awareness about the importance of maintaining psychological well-being and to reduce the prevalence of psychological symptoms.

Resumen: La idiosincrasia de la profesión médica, los rasgos de personalidad característicos del médico y la falta de formación específica para reconocer y tratar adecuadamente la propia vulnerabilidad, predisponen a este colectivo a padecer más patología mental y, probablemente, un deficiente tratamiento de otras enfermedades. La mayoría de los estudios realizados hasta el momento se centran en la patología mental y adictiva del médico, desde el punto de vista del riesgo para la mala praxis y la seguridad de sus pacientes. La revisión narrativa de la literatura científica realizada (MEDLINE, EMBASE e IME 1985-2016) ha mostrado que, en el entorno hispanoparlante, apenas disponemos de información actualizada sobre el médico como paciente, a pesar de ser un tema de indudable relevancia desde el punto de vista de la seguridad asistencial, del profesionalismo y del propio bienestar de los profesionales. La situación del médico que enferma es compleja y poco conocida, con conflicto de roles y con repercusiones en la ética profesional y la calidad asistencial. Palabras clave: enfermedad, médico, estudiante de medicina, médico residente, jubilación, automedicación, autotratamiento, acceso a la atención, programas de salud, salud mental, ética médica, deontología profesional The process of physicians falling ill and consequences for their professionalism Abstract: The idiosyncrasy of the medical profession, the characteristic personality traits of the physician and the lack of specific training to properly recognize and treat one's own vulnerability dispose this group to suffer more mental pathology and probably worse control of other medical diseases. Most of the studies promoted so far focus on the psychiatric and addictive concerns of the physician, from the point of view of the sick doctor as being at risk for malpractice and to guarantee the safety of their patients. The narrative review of scientific literature (MEDLINE, EMBASE and IME 1985-2016) has shown that we do not have in the Spanish-speaking environment updated information about the doctor as a patient, despite being a topic of incipient media repercussion and undoubtedly relevant from the point of view of patient safety, professionalism and medical ethics and as well as the welfare of professionals. The characteristic of a sick doctor are more complex than the rest of patients with a conflict of roles and repercussions on professional ethics and the quality of care.

INTRODUCTION In New Zealand, 41% of general practitioners (GPs) intend to retire by 2025. Increasing workforce shortages and other stressors are putting doctors at risk of burnout, which in turn can put patients at risk of harm. Offering a range of resources can signal an organisation's commitment to physician wellness while improving patient safety and organisational stability. AIM To replace the current reactive approach to impaired doctors with a proactive system of monitoring performance with the goal of identifying problems early. METHODS This paper reports on an initiative of Pegasus Health Charitable to provide pastoral care to GPs in Canterbury experiencing increased stress, burnout or problems leading to impaired performance. RESULTS The pastoral care programme has been running successfully for 9 years and has helped 32 GPs. Because of the low numbers, the programme needs to be individualised and confidential. CONCLUSION Recent developments have seen Pegasus Health adopt a systematic approach to monitoring and supporting health practitioners. This includes the monitoring of available data on GPs at risk. Data collection is being used to manage the "psychological health" of doctors, including complaints, prescribing, referral data and attendance at education sessions.
