How to Define Cardiac Resynchronization Therapy Response

Abstract: ABSTRACT. Cardiac resynchronization therapy (CRT) improves quality of life, exercise capacity, and cardiac function in a significant number of patients with heart failure (HF) and left bundle branch block. CRT also reduces HF hospitalizations and overall mortality. A substantial number of patients however do not have a favorable clinical response. Evaluation of CRT non-response has been challenging as the primary mechanism for CRT improvement remains elusive. In addition, reported CRT response rates have varie… Show more

“…The definition of CRT response has varied in previous clinical trials [ 6 , 7 , 8 ]. However, the most common definition of CRT response includes echocardiographic parameters describing reverse remodeling [ 12 , 13 , 14 ], perhaps because echocardiographic data are more objective than symptoms, and some patients feel better after treatment and may experience placebo effects.…”

“…However, we believe that patients in actual clinical practice have a better response than that reported in the literature. There are various definitions of CRT response, and the response rate varies significantly according to the definition [ 6 , 7 , 8 ]. In general, two definitions have been used in most clinical trials, i.e., echocardiographic response and functional response.…”

“…However, it should be acknowledged that the reported response rates vary with the criteria used to assess the response. 20 The relatively small number of patients as well as the retrospective setting are limitations to this study particularly concerning our observations regarding CRT responses.…”

“…1 However, there is a lack of standard definition for CRT response and the spectrum of CRT response and reported response rates vary widely depending on the metrics used and whether or not a placebo effect is considered. [2][3][4] Regardless of the definition considered, however, nonresponse to CRT remains a significant problem. This manuscript presents five cases of nonresponse to CRT and will discuss the approaches used to overcome this problem in each.…”
