How Patchy Is Patchy Villous Atrophy? Distribution Pattern of Histological Lesions in the Duodenum of Children With Celiac Disease

Abstract: In newly diagnosed CD, some variability of histological lesions can be found, even within the same duodenal biopsy, in which areas of apparently normal mucosa with increased IEL number often exist. We also confirm our previous findings that duodenal lesions may vary among different biopsies; lesion severity has a proximal-to-distal gradient, but no patient has entirely normal duodenal biopsies. The awareness of such histological variability may help establish a correct diagnosis of CD.

“…Allergic colitis may mimic UC particularly in infants, but also in children beyond infancy (95). Eosinophilic gastroenteritis after infancy may mimic CD with ulcerations, skip lesions reaching from the stomach to the colon and, unlike its infantile counterpart, is uncommonly associated with allergy (96).…”

“…16 In addition, diagnostic studies rely on multiple duodenal biopsies, as villous atrophy can be patchy; yet clinical practice regarding the number of biopsies is not uniform. 19,20 Consequently, local differences in practice may explain discrepancies between near-neighbor probability estimates and those derived from published studies. Furthermore, near-neighbor analysis reports continuous likelihood where most published studies use only binary cutoffs, so the greatest discrepancies are often near cutoff values.…”

“…The abnormal histological features of the small intestine observed in celiac disease may show variable severity and could be patchy with signs of injury only in certain parts of the small intestine [19]. Therefore, during upper endoscopy it is recommended to collect at least one bulb biopsy and four biopsies from the more distal part of duodenum [14] and some clinicians are encouraged to avoid collecting bulb biopsies due to that the presence of Brunner´s glands can complicate the pathological evaluation [20].…”
