How I diagnose and treat neonatal thrombocytopenia

Abstract: Neonatal thrombocytopenia defined as the presence of a circulating platelet count <150x109/L is a common abnormality in babies admitted to Neonatal Intensive Care Units. Thrombocytopenia that is typically mild and self-limiting often accompanies neonatal stress in scenarios such as premature delivery or intrauterine growth restriction. However, the differential diagnosis of neonatal thrombocytopenia is wide and includes potential life-threatening disorders such as bacterial sepsis, viral infection and n… Show more

“…Thrombocytopenia is the most frequent hematological issue found in newborns, excluding anemia caused by phlebotomy [12,13]. It is particularly common in NICUs, which might lead some to dismiss it as a non-issue and simply administer platelet transfusions when the count drops below a certain level [14][15][16].…”

“…The incidence of thrombocytopenia in neonates admitted to the neonatal intensive care unit (NICU) can range from 22-35% 2,3 . Chronic fetal hypoxia resulting from pregnancy-induced hypertension (PIH) is the primary cause of earlyonset neonatal thrombocytopenia within 72 hours after birth [4][5][6] . PIH comprises gestational hypertension, preeclampsia, and eclampsia, which can cause an imbalance in the blood systems of mothers and neonates [7][8][9][10][11] .…”

“…Intracranial hemorrhage (ICH) is an important cause of neonatal morbidity and mortality (6). IHC can be particularly harmful in the neonatal period as this is a critical stage for brain development (7,8). Reported cases of WAS that start with ICH are rare.…”
