House dust mite sublingual immunotherapy: a double‐blind, placebo‐controlled study in elderly patients with allergic rhinitis

Abstract: Sublingual allergen-specific immunotherapy in elderly patients with a HDM allergy to D. pteronyssinus and D. farinae generated a significant clinical improvement in the active group compared with the placebo group, particularly during the heating season. This therapy was well tolerated.

“…19 In another field trial of HDM sublingual drops in 108 elderly subjects, onset of action was not formally assessed, but clinical effect on nasal symptoms appeared to begin at approximately 6 months. 20 Thus, the onset of action of 12 DU of MK-8237 appears to be almost twice as fast as has been reported for HDM SLIT in other trials.…”

“…However, there are several contraindications for AIT including comorbidities, such as autoimmune diseases and malignancies, concomitant drug treatments, particularly β-blockers and angiotensin-converting enzyme inhibitors (ACEI), pregnancy for induction therapy, and, currently, severe asthma [60,61]. There are also some concerns about the age limits for AIT use, although elderly people have shown good tolerance and clinical improvements with SLIT for mite allergy [62,63]. These concerns also affect to very young children but this topic will be discussed in a section below (see AIT efficacy in pediatric populations).…”

“…AIT is seldom offered to older patients due to safety concerns and the perception that the allergic disease is already 'too advanced' and unlikely to benefit from AIT. A recent study has demonstrated significant clinical efficacy of HDM SLIT in subjects of age >60 years who had a long history of perennial HDM AR [74]. Another recent double-blind placebo-controlled study from the same group evaluating the clinical efficacy of grass pollen SLIT (78 allergic subjects aged 60-70 years) reported 64% decrease in nasal symptoms versus 7% with placebo [75].…”
