Background: Various studies have drawn similarities between respiratory conditions such as pneumonia and COVID-19. Respiratory disease (e.g., viral pneumonia J12.8) or signs or symptoms of respiratory disease (e.g., shortness of breath R06.0, cough R05) a respiratory disease (e.g., viral pneumonia J12.8) are the most notable symptoms that are associated with COVID-19. Pneumonia is also among the top five most prevalent reasons for admissions in medical schemes and is one of the expensive conditions to treat. This further accounts for a significant portion of hospitalisation expenditure. Objectives:The primary objective of this study was to study the characteristics of medical scheme patient's viral pneumonia as a primary ICD-10 diagnosis and ICD-10 secondary diagnosis that were admitted into private hospitals. Methods:The study design was a descriptive analysis of medical schemes admissions data that occurred in 2020. The review period was claims received between March to August 2020. The inclusion criteria were patients viral pneumonia primary ICD-10 code. Patients with a secondary ICD-10 code who had a COVID-19 related diagnosis were patients who had a laboratory-confirmed (RT-PCR assay) COVID-19. Results:The study covered a total of 53 medical schemes' data. These accounted for 8,3 million lives. The analysis included a total of 3,437 admissions that had a viral pneumonia primary admissions ICD-10 code. Other viral pneumonia accounted for a significant proportion of the admissions; 82 percent. The second-highest group was viral pneumonia, unspecified, which accounted for 16 percent of admissions. Other types of viral pneumonia included respiratory syncytial virus pneumonia, adenoviral pneumonia and parainfluenza virus pneumonia, which accounted for the remaining 2 percent. The paid amount for other viral pneumonia compared to viral pneumonia, unspecified was not statistically significant, (R66 399 vs. R67 490; p = 0.8961). The average inpatient days were also not significantly different (9.3 days vs. 8.7 days p = 0.1679). Other viral pneumonia compared to viral pneumonia, unspecified were likely to have a COVID-19 confirmed secondary ICD-10 diagnosis, 63 percent versus 44 percent, p<0.001. Significantly, less other viral pneumonia compared to viral pneumonia had a suspected COVID-19 secondary ICD-10 admissions diagnosis, 2 percent vs. 18 percent, p<0.001. Lastly, significantly more other viral pneumonia compared to viral pneumonia had essential primary hypertension COVID-19 secondary ICD-10 admissions diagnoses, 5 percent vs. 2 percent, p<0.001. Conclusion:The study found that mainly the other viral pneumonia and the viral pneumonia, unspecified primary ICD-10 diagnoses were more likely to be admitted into hospitals. The study found that other viral pneumonia had a higher likelihood than viral pneumonia, unspecified to have a COVID-19 secondary ICD-10 admission diagnosis and were more likely to be associated with a COVID-19 admissions diagnosis. Pneumonia infects the respiratory system, especially the lungs, and can potentially cause an epidemic. This problem was fourth highest number as an inpatient case at the RSU Haji Medan in 2023. Aim of this study was to determine the factors associated with length of stay (LOS) in management pneumonia. This analytical study was combined with cross-sectional design. This study recruited 94 patients by the purposive sampling technique. All data of this study extracted from medical records. The results of this study showed that improved clinical status due to indications of comorbidities such as blood sugar(P=0,000), blood pressure(P=0,007), and hemoglobin(P=0,034) levels were risk factors for the length of stay (LOS) of pneumonia patients. This study showed that the Length of stay (LOS) for pneumonia patients was associated with complications of diabetes, hypertension, and anemia. Suggestions that can be given are dietary monitoring through nutrition facilitators, then initiation of therapy for hypertension and anemia patients such as giving anti-hypertensive medications, vitamin B12 supplementation to blood transfusions. Masalah ini merupakan masalah tertinggi keempat sebagai kasus rawat inap di RSU Haji Medan pada tahun 2023.Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang berhubungan dengan lama rawat inap (LOS) dalam penatalaksanaan pneumonia. Penelitian analitik ini merupakan penelitian potong lintang dengan desain cross sectional. Penelitian ini merekrut 94 pasien dengan teknik purposive sampling. Semua data penelitian ini diambil dari rekam medis. Hasil penelitian menunjukkan bahwa peningkatan status klinis akibat indikasi penyakit penyerta seperti peningkatan gula darah sewaktu (P=0,000), tekanan darah tinggi (P=0,007), dan kadar hemoglobin yang rendah (P=0,034) merupakan faktor risiko terhadap lama rawat inap pasien pneumonia. Penelitian ini menunjukkan bahwa Length of stay (LOS) pasien pneumonia berhubungan dengan komplikasi diabetes, hipertensi, dan anemia. Saran yang dapat diberikan adalah pemantauan pola makan melalui fasilitator gizi, kemudian inisiasi terapi bagi pasien hipertensi dan anemia seperti pemberian obat anti hipertensi, suplementasi vitamin B12 hingga tranfusi darah. Objectives The incidence of hospitalization in Hajj pilgrims can be influenced by age, physical activity, and comorbidities, such as hypertension and diabetes melitus. Smoking, low levels of physical activity, cardiovascular, respiratory and endocrine metabolic diseases are the risk factors of mortality for Indonesian pilgrims. This study aims to identify the relationship between risk factors owned by pilgrims with hospitalization and mortality that occurs during the Hajj period. Methods This was a cross-sectional, non-intervention study with data retrieved from the pre-embarkation Hajj screening records. The diagnosis of hospitalization and cause of death during the Hajj period were obtained from the medical report and hospital/flight doctor death certificate. Results A total of 2933 eligible subjects were analyzed in this study, with 11 subjects having a history of hospitalization. The 11 subjects were over 50 years old with male (n=6, 54.5%) and women (n=5, 45.4%) having almost the same number. Bivariate analysis for comorbidities and laboratory outcomes resulted in diabetes melitus as the only comorbidity that increases the risk for hospitalization in Hajj pilgrims (p=0.035). There was no association between existing comorbidities with mortality during the Hajj period. Multivariate analysis using logistic regression showed that higher white blood cell levels were associated with higher risk of both hospitalization (p<0.001) and mortality (p<0.001) with mean value of 8030 ± 5290 mg/dl. Conclusions The risk factors associated with the hospitalization rate of pilgrims in Indonesia are a history of diabetes mellitus and higher white blood cell levels. Pilgrims with higher white blood cell levels with a mean value of 8030 ± 5290 mg/dl also have an increased risk of mortality during Hajj period.
