Hospital admission medication reconciliation in medically complex children: an observational study

Abstract: In children with MCC admitted at our institution during the study period, no medication information source was optimally available, sensitive and specific. Admitting order medication errors affected more than half of patients, the most common being omissions. Efforts to improve medication reconciliation at hospital admission in this population must account for availability and accuracy of information sources and medication omissions at the time of hospital admission.

“…All medications have the potential for adverse effects, and the use of multiple medications increases the risk of drugdrug interactions. 27 Maintaining current medication lists for the caregivers with clear dosing regimens is essential, and updated lists must be maintained in the medical home. This action facilitates timely refills, reduces risks of medication errors, and allows health care providers to regularly monitor for and address potential drug interactions.…”

“…Outpatient pediatric polypharmacy has not been adequately described; in a study examining medication reconciliation for children with CCCs, subjects took, on average, 5 chronic medications. 29 Polypharmacy, whether necessary for successful treatment of a patient, has been associated with increased risk of drugdrug interactions or ADEs. 30 Complex medication regimens may also be difficult to manage for both caregivers and clinicians.…”

“…Il faut toutefois noter qu'il Bien que la majorité des enfants reçoivent moins de médicaments que les adultes, la démarche de BCM à l'admission est tout aussi importante pour ce groupe d'usagers [24][25][26] . Pendant la durée de l'étude, les BCM à l'admission comportaient majoritairement un ou aucun médicament.…”
