Homogeneous Blue Pattern in an Acral Congenital Melanocytic Nevus

Abstract: Dermoscopy in acquired acral melanocytic nevi has been widely studied. This is in contrast with the little information about the dermoscopic characteristics in congenital acral melanocytic nevi. We report a 46-year-old man who was referred due to a lesion on his right sole present since childhood corresponding to an acral congenital nevus. Dermoscopy showed a homogeneous blue pattern not previously described in acral benign tumors. The homogeneous blue pattern has previously been associated with blue nevus and… Show more

“…Previously, the homogeneous blue pattern and the multicomponent pattern con- sisting of the parallel furrow, latticelike, and fibrillar patterns were documented as dermoscopic features of acral CMN. 17,18 In our series, 15 lesions exhibited the parallel furrow pattern, among which 6 showed only that pattern. In 8 lesions, the parallel furrow pattern was combined with the crista dotted pattern, and 1 lesion showed a combination of the parallel furrow, crista dotted, and fibrillar patterns.…”

“…4 These findings were not corroborated in our study, as in most cases for both types of lesions (blue naevus and blue naevus-like CMMM) there were no vascular structures within the homogeneous blue pattern; likewise in a small proportion of both types of lesion, vessels were present and nonspecific. Further-more, some congenital acral naevi have a homogeneous blue pattern, as reported by Garrido-R ıos et al 17 Samimi et al 18 reported that ultrasonography is a reproducible tool that can assist in the differential diagnosis between blue naevus and melanoma metastases, but there should be a clinical suspicion present.…”

“…Dermoscopic patterns of CAMN have been described only in a case series of Asian patients and in two case reports. [6][7][8] To our knowledge, there has been no previous report comparing CAMN and AAMN. Our study cohort showed that the parallel furrow pattern was the major dermoscopic pattern in acral naevi in the paediatric population, irrespective of the congenital or acquired character.…”
