Child maltreatment (CM) has been enormously studied. However, a preventive practice still requires comprehensive and effective instruments to assess the risks for CM in a family context. The aim of this study is to describe the development process of an evidence-based CM risk assessment instrument (Family Needs Checklist, FNC) for primary prevention online utilization. This article reports the development process of the checklist and its mobile application, consisting of a systematic literature review, identification of known risk factors using the content analysis method, and generation of the checklist, including a multidisciplinary group in the design and feedback. As a result, a comprehensive and compact checklist was developed to be used by parents or caregivers as a self-referral instrument with an option to be used with professionals as a basis for joint conversations. The FNC consists of parental, family-, and child-related risk factors. Based on the international evidence, the online application consists of knowledge about different CM types, information about risk factors and protective factors as well as recommendations and guidance to support services. The FNC is based on robust evidence on known risk factors causing CM in families. It can be used for primary prevention utilization in the general population.

Factors Associated with Child Maltreatment Fatality among Young Children with an Open Child Protective Services Case at Death

One of the goals of Child Protective Services (CPS) is to prevent maltreatment deaths. We analysed deaths of children younger than five years of age with an already open CPS case at the time of death for characteristics that might be associated with increased risk of fatality with child maltreatment. We used 2513 cases in the National Center for Fatality Review and Prevention – Case Review System during the years 2004–2016 for children ages 0–4 years who died with an already open CPS case. Among these, we compared 1370 children who died from causes determined to be ‘caused or contributed to’ by abuse or neglect to 1143 children who died from other causes. Those who died from maltreatment were older, experienced prior maltreatment, lived in an overcrowded residence, and had a caregiver with history of substance abuse, interpersonal violence, criminal delinquency and their own maltreatment. These risk factors and others may be associated with a higher likelihood of dying with abuse or neglect for children who already have an open CPS case. Key Practitioner Messages Compared with children dying from other causes, children with an already open CPS case at the time they died as a result of maltreatment were more likely to be older, male and living in a parental home with overcrowding. Caregivers were more likely to be male, or had a history of substance abuse, delinquency or prior maltreatment, both as a victim or perpetrator. When cases are opened with CPS, caseworkers should consider these potential risk factors and whether additional services can be put in place to reduce future maltreatment fatality. While additional research is needed, we speculate that other services, such as coordinated paediatric care, universal reporting, child fatality review teams, citizen review panels and public education campaigns, can offer additional ways to decrease future maltreatment deaths.

Filicides, where children are killed by their parents or stepparents, are uncommon occurrences, which are committed by both paternal and maternal perpetrators. The aim of this paper is to explore to what extent the antecedents of filicides can be traced back to one of the factors identified within the literature the adverse childhood experiences of the perpetratorsand what this might mean for the way that services seek to intervene to safeguard and support children in precarious situations. Systematic searches were used to retrieve relevant articles in six electronic databases: AMED, CINAHL, Criminal Justice Abstracts, Medline (PubMed), PsycINFO and SCOPUS. Key findings were that numerous studies reported significant levels of complex, multiple and sustained experiences of childhood adversity for many perpetrators, and that various potential pathways to filicide perpetration may emanate from such experiences. This review suggests that evidence-based interventions should be made readily available early in life to persons experiencing adverse childhood experiences, together with supportive services to those who become parents and carers. Such support may help prevent the compounding of adversity over time, thereby reducing the potential for risk of harm and possible tragic outcomes for their dependent children.
