Home Health Care and Nursing Education: Creating Community Practice Models of the Future

Abstract: The restructuring of the health care system will result in expansion of health promotion and disease prevention services in the community. Nurses in education and home health care can work together to develop practice models of the future by using the resources of both institutions to improve the community's health. The Neighborhood Wellness Center, a nurse managed center, is described and the benefits to home care, nursing education, and the community are discussed.

As home health care takes a more visible stance in prenatal and postpartum care, health care providers will be called upon to be resources of information regarding these therapies. Popular holistic therapies for insomnia, weight loss, and fatigue will be discussed as well as their implications on the fetus and nursing infant.UCH CONCERN is generated from be-t~~ hind the counter when a breastfeeding t ~~~ mother or mother-to-be reaches to the m V &dquo;miscellaneous&dquo; section of over-thecounter (OTC) drugs. She may be looking for her answer to insomnia, weight loss, or fatigue. In most retail pharmacies, the miscellaneous section consists of herbs, vitamins, powdered drinks, weight loss remedies, and the most recent panaceas. Background: The phenomena experienced by professional nurses who make practice-based career changes are seldom addressed in the literature. Nurses changing from a hospital-based practice to a home health care setting report feelings of anxiety, incompetency, and lack of the necessary skills to care for clients in the home. Method: An integrative review of the literature on role theory can provide the conceptual understanding of the transitional experience of nurses whose roles change when they move from hospital-based practice to a home health care setting. Results: A model of the role transition process is helpful in identifying the transition experienced by nurses new to the home health care setting. Experiences during the initial transition period are critical in shaping the nurse's understanding of the role. Conclusion: Educators can play a key part in assisting novice home health care nurses with role enactment by developing comprehensive orientation and education programs aimed at minimizing role strain.
