Holmiumlaser versus mechanische knorpelabtragung

Abstract: The aim of this work was to investigate and evaluate the tissue reactions after sham operation and after mechanical and holmium laser chondral debridement in a rabbit model at different postoperative intervals. In 27 rabbits an arthrosis was induced by implantation of a piece of polyethylene sheeting in one knee joint. After 5 months, distinct arthritic changes had developed. Nine animals each were randomized for sham operation, mechanical debridement, and holmium laser chondral debridement. At 1 day, 1 month,… Show more

“…These results conform to the few clinical studies on cartilaginous laser application [2,4,7,15,22,29]. On the other hand, there are several experimental studies with contradictory results [7,9,18,20,23,28,32]. Because the laser was applied very carefully and tangentially in an unfocussed system in this study, unlike some experimental studies with negative results, comparison is not possible.…”

“…After conventional arthroscopy with partial meniscal resection an increased load transmitted to the already injured cartilage has been hypothesized to result in subchondral microfractures or changes in the subchondral blood supply, typically leading to avascular necrosis in the elderly [19, 20]. Although laser‐assisted surgery‐induced osteonecrosis as a result of thermal or photoacoustic tissue damage to the underlying matrix has been described, few reports have considered that a similar effect could result from the use of RFE devices [12, 13, 21, 22]. Most recently, Bonutti and colleagues presented a series of nineteen patients treated with knee arthroplasty for osteonecrosis.…”
