Przegląd Dermatologiczny 2016/3 STRESZCZENIEWprowadzenie. Pierwotnie skórny anaplastyczny chłoniak wielkokomórkowy (ang. cutaneus anaplastic large-cell lymphoma -C-ALCL) należy do rozrostów limfoproliferacyjnych z komórek T CD30+. Przebiega zwykle łagodnie i nie wymaga agresywnego leczenia. Rokowanie w C-ALCL jest dobre, a 10-letnie przeżycie osiąga 90% chorych. U 1/3 pacjentów możliwe są samoistne remisje choroby. Cel pracy. Przedstawienie pacjentki z wywiadem nawrotowej ziarnicy złośliwej, u której rozpoznano pierwotnie skórnego chłoniaka anaplastycznego wielkokomórkowego. Opis przypadku. Pacjentka 35-letnia z wywiadem nawrotowej ziarnicy złośliwej została przyjęta do Kliniki w celu rozpoznania i leczenia zmian skórnych utrzymujących się od kilku tygodni. Na podstawie badania histopatologicznego i immunohistochemicznego rozpoznano pierwotnie skórną postać chłoniaka T-komórkowego. Po 2 tygodniach obserwowano spontaniczną regresję zmian skórnych. Wnioski. Pacjenci z anaplastycznym chłoniakiem wielkokomórko-wym wymagają pogłębienia diagnostyki i ze względu na agresywny przebieg i odmienne postępowanie terapeutyczne wykluczenia postaci układowej choroby, przerzutu chłoniaka Hodgkina do skóry oraz transformacji wielkokomórkowej ziarniniaka grzybiastego. ABSTRACT Introduction.Cutaneous anaplastic large cell lymphoma (C-ALCL) is one of the CD30+ lymphoproliferative disorders. Usually it has a mild course and does not require aggressive treatment. The prognosis in C-ALCL is good, with the 10-year survival rate reaching 90% of patients. Spontaneous remissions of the disease are possible. Objective. To present a patient with recurrent Hodgkin's disease, in whom primary cutaneous anaplastic large cell lymphoma was diagnosed. Case report. A 35-year old woman with recurrent Hodgkin's disease was admitted to the department for diagnosis and treatment of skin lesions, which appeared a few weeks ago. Primary cutaneous T-cell lymphoma was diagnosed by histopathology and immunohistochemistry. After two weeks spontaneous regression of skin lesions was observed. Conclusions. Larger diagnostic procedures and exclusion of the systemic form of ALCL, cutaneous metastases of Hodgkin's disease and large-cell transformation of mycosis fungoides are required due to the aggressive course and different treatment in all cases.Pierwotnie skórny anaplastyczny chłoniak wielkokomórkowy u pacjentki z ziarnicą złośliwą -opis przypadku

“…Theoretically, two bands should be observable in PARR-SSCP of a monoclonal lymphoid tumor sample because each double-strand DNA molecule separates into 2 single strands [5]. However, in some reports, several bands have been observed in PARR-SSCP in addition to the target bands, although the cause of this is unknown [3,6,8,10]. Although we could not elucidate the reason for this, our results show that SSCP analysis helps in determination of PARR clonality in samples showing an obscure band in conventional PARR.…”

ABSTRACT. PCR for antigen receptor gene rearrangement analysis (PARR) is a new diagnostic method for lymphoid neoplasia. In PARR using formalin-fixed paraffin-embedded tissues (PARR-FFPE), control DNA amplification was successful in only three of five samples. The formalin fixation times of the three samples were shorter than those of the others. Analysis of the formalin fixation time and DNA amplification controls suggested that a formalin fixation time of less than one week is appropriate. Additionally, application of single strand conformation polymorphism (SSCP) for PARR provided clearer results than conventional PARR in 16 unfixed tissues and three FFPE tissues. These results show that PARR-FFPE is viable for tissues with an appropriate formalin fixation time and that application of FFPE and SSCP for PARR are useful for diagnosis and retrospective study of canine lymphoid neoplasia.
