HLA B27 and the genetics of ankylosing spondylitis.

Abstract: SUMMARY One hundred and twenty-eight of 145 patients with ankylosing spondylitis (AS) were found to be HLA B27 positive. Five patients had evidence of a sero-negative peripheral arthritis resembling peripheral psoriatic arthritis and 3 of these were B27 negative. One further B27 negative patient had a sister with ankylosing spondylitis and ulcerative colitis and a mother with ulcerative colitis. There was evidence of a somewhat later age of onset of symptoms in B27 negative patients. These findings are interpr… Show more

“…An increased frequency of overt IBD was observed in the families with B27-negative patients. This is consistent with previous descriptions of HLA-B27-negative familial SpA (3,25,26) as well as with the suggested sharing of predisposing factors between IBD and SpA (26). We also found an increased frequency of the HLA-B35 allele in these families, which has previously been observed in AIBD (28) as well as in HLA-B27-negative SpA (ref.…”

“…Finally, HLA-B27 was present in all 47 of the patients tested for this antigen, thus confirming that the close association between definite AS and HLA-B27 also includes women with this disorder (19)(20)(21)(22). In regard to genetic susceptibility, 15 of our 50 patients had close relatives with definite AS, and 5 of those IS had more than I affected relative.…”
