HIV policy implementation in two health and demographic surveillance sites in Uganda: findings from a national policy review, health facility surveys and key informant interviews

Abstract: BackgroundSuccessful HIV testing, care and treatment policy implementation is essential for realising the reductions in morbidity and mortality those policies are designed to target. While adoption of new HIV policies is rapid, less is known about the facility-level implementation of new policies and the factors influencing this.MethodsWe assessed implementation of national policies about HIV testing, treatment and retention at health facilities serving two health and demographic surveillance sites (HDSS) (10 … Show more

“…Progress with implementation of national HIV policies has also been evaluated in a cross-sectional study of six health facilities in Karonga, Malawi, in December 2013 [ 53 , 54 ]. Free HTC, PMTCT and ART services were provided at all facilities and Option B+ was available in 90% of facilities, suggesting slightly earlier implementation of these services in Malawi compared to east Zimbabwe.…”

“…Common qualitative methods utilized across the studies included focus group discussions and key informant interviews. Mixed methods studies used review of financial records [15], routine facility, or surveillance indicators [13,17,22,28,43], health worker questionnaires or other quantitative study process indicators [10,20,23,28,29], or validated surveys to calculate measures such as organizational readiness and provider burnout [24] in conjunction with qualitative research. CFIR constructs can be scored quantitatively and compared across cases according to strength and valence [44].…”

“…We also need data about implementation gaps at the facility level and the consequences for managing supplies of drugs and diagnostic tests. Although previous research has investigated HIV policy adoption and implementation in sub-Saharan Africa, 12,[15][16][17][18][19] few studies have documented implementation of PMTCT policies, especially after implementation of Option B+, and in rural settings. This paper used data from policy reviews and health-facility surveys conducted with health workers between 2013 and 2016 in five health and demographic surveillance system sites in rural Malawi, South Africa and the United Republic of Tanzania.…”
