“…HIV-specific systemic (blood) responses have been previously described in our study cohort of HIV-1-resistant sex workers (15,16); however, the vast majority of HIV-1 transmission in this population occurs across the genital mucosa during heterosexual intercourse (25). Therefore, HIV-specific CTL in the cervix might play a more direct role than do systemic responses in immunemediated protection against sexually acquired HIV-1 infection.…”

“…In an observational cohort study of the epidemiology and immunology of HIV-1 in sex workers in a slum area of Nairobi (24,25), we have identified a subgroup of women who exhibit relative resistance to HIV-1 infection (26). It is estimated that these women have over 60 unprotected sexual exposures to HIV-1 per year, despite behavioral counseling and provision of condoms; their exposure is uniformly through vaginal intercourse.…”

“…The finding of IgA-mediated inhibition of HIV-1 transcytosis therefore suggests that these highly exposed, seronegative women may have developed an array of mucosal immune defenses to protect against sexual HIV-1 acquisition. It is of interest that IgA with the ability to inhibit HIV-1 transcytosis was not found in the saliva of these sex workers, because oral sex is quite uncommon in the sex worker cohort (29), while IgA purified from the saliva of seronegative subjects in the Italian discordant couple cohort was often able to inhibit transcytosis. However, it is also possible that this is a reflection of the different methods used to collect saliva specimens in the two cohorts.…”

“…Sex workers were enrolled from a dedicated sex worker clinic in Nairobi, Kenya (9), between 1995 and 1999. Seronegative sex workers were subdivided into three groups, based on the duration of previous HIV-1-uninfected follow-up in the cohort: those enrolled for over 3 years (thereby meeting criteria for HIV-1 resistance; group 1) (11), those enrolled for 1-3 years (group 2), and sex workers followed for less than 1 year (group 3).…”
