Histopathologic parameters in chronic rhinosinusitis with nasal polyposis: impact on quality of life outcomes

Abstract: For most patients QOL significantly improved after surgery. Mucosal eosinophilia did not correlate with the absolute change of the RSDI. However, thickening of basal membrane adversely affects symptoms of the patients and correlates with the disease severity.

“…Eosinophils are a major inflammatory cell in eosinophilic rhinosinusitis, 36 a more severe form of sinonasal inflammation that is proving difficult to treat. 37,38 Mast cells were found throughout the OE epithelium after LL-37 treatment in a dose dependent manner. Interestingly, both LL-37 and mast cells are both highly elevated in patients with sinonasal inflammation.…”

“…The presence of goblet cells and subepithelial thickening in perioperative specimens from CRS patients correlates with some bothersome nasal symptoms after ESS [49]. The thickening of the basal membrane adversely affects the patients' quality of life, as measured by the Rhinosinusitis Disability Index (RDI) and the Short Form-36 (SF-36) General Health Survey quality of life questionnaires, and correlates with the disease severity in patients with CRS [50]. However, although information that is obtainable from light microscopy can serve as valuable prognostic indicators, routine H&E staining is suboptimal in distinguishing among the various CRS subgroups [79].…”
