In the past decades, the incidence of melanoma has been reported to rise in epidemic proportions. The chief reason for that pseudo-epidemic is improved criteria for diagnosis that allow melanomas to be recognized far more accurately and at earlier stages. The rising number of melanomas diagnosed has resulted in increased diagnostic scrutiny, more pigmented lesions being biopsied and more melanomas recognized, thus enhancing the 'epidemic' in self-perpetuating fashion. Regression of melanomas may, in part, explain why lesions undetected before did not result in a far higher mortality. Another potential reason for the disparity between increasing incidence of melanoma and relatively steady mortality may be overdiagnosis of melanoma. The latter may be curtailed by establishment of well-defined diagnostic categories, efforts to establish reliable criteria for recognition of those categories, better clinicopathologic correlation, postponement of biopsy of pigmented lesions in the case of irritation and excisional rather than incisional biopsies.

Study Aim: Cutaneous melanoma accounts for approximately 4% of all skin cancers diagnosis each year in the USA but is responsible for 75% of skin cancer deaths. Physicians need to have an up to date knowledge about the epidemiology, risk factors, and clinical features of melanoma in order to make an early diagnosis and provide appropriate management decisions to improve survival in this aggressive malignancy. We will be presenting an extensive, up to date, literature review of cutaneous melanoma including the epidemiology, risk factors, diagnosis and staging. Method: An extensive literature review was performed using the following search engines: Pubmed, Ovid, Miromedex, and Dynamed. We limited our literature search to research papers written in English using the following keywords: cutaneous melanoma, epidemiology of melanoma, risk factors for melanoma, diagnosis of melanoma, clinical classification and staging of melanoma. Results: As a result of our extensive literature review we present an up to date review on the epidemiology, pathogenesis, risk factors, diagnosis, classification and staging of cutaneous melanoma. Conclusion: Physicians need to have an up to date knowledge about the epidemiology, risk factors, and clinical features in order to make an early diagnosis and provide appropriate management decisions in order to improve the survival patients with this aggressive malignancy.

Impact ofin vivoreflectance confocal microscopy on the number needed to treat melanoma in doubtful lesions

Background The number needed to treat ratio is an effective method for measuring accuracy in melanoma detection. Dermoscopy reduces the number of false positives and subsequently unnecessary excisions. In vivo confocal microscopy is a non-invasive technique which allows the examination of the skin with cellular resolution. Objectives To assess the impact of RCM analysis on the number of equivocal lesions, assumed to be melanocytic, excised for every melanoma. Methods Consecutive patients (n=343) presenting with doubtful lesions, were considered for enrolment. The lesions were analysed by dermoscopy and RCM and histopathological assessment was considered the reference standard. The main outcome was the number needed to treat, calculated as the proportion of equivocal lesions, excised for every melanoma. Results Dermoscopy alone obtained a hypothetical NNT of 3.73, the combination of dermoscopy and RCM identified 264 equivocal lesions that qualified for excision, 92 of which were confirmed to be a melanoma; resulting in a NNT of 2.87; whereas the analysis of RCM images classified as melanoma 103 lesions with a consequent NNT of 1.12; the difference in the reduction of this ratio was statistically significant (p< 0.0001) between the three groups. There was no significant improvement in sensitivity when comparing the combination of dermoscopy and RCM and RCM alone (94.56% vs. 97.82%; p = 0.043). However, the differences between specificities were statistically significant (p <0.000001), favouring RCM alone. Conclusion The addition of RCM analysis to dermoscopy reduces unnecessary excisions with a high diagnostic accuracy and could be a means for reducing the economic impact associated with the management of skin cancer.
