Histologic Characteristics of the Human Prepuce Pertaining to its Clinical Behavior as a Dual Graft

Abstract: The aim of this study was to investigate the unique histologic structure of the normal human prepuce, paying particular attention to the resemblance and dissimilarities between the inner (ie, mucosa) and outer (ie, skin) layers. Histologic sections were stained using hematoxylin-eosin and Van Gieson stains. Transmission electron microscopy was used to evaluate the ultrastructure. Dense capillary networks can be observed in both the upper and lower dermal zones. The dermis lacks a dense collagenous zone. Melano… Show more

“…[16] It is a simple fold of skin composed of an outer keratinized (skin) and inner (mucosal, lining a preputial sac) layers [Figure 1a] that are rich in nerves. [1718] The abundance of myelinated and nonmyelinated nerve fibers explains the high sensitivity of the human foreskin and its function as erogenous tissue. [1518]…”

“…The prepuce (foreskin) is richly innervated with sensory neurons, explaining the high sensitivity of the human foreskin (24). The precept behind circumcision to treat PE is that the surgical removal of the prepuce also ablates many of these sensory receptors, thus decreasing glans sensitivity.…”

“…However, the foreskin is a richly innervated structure that protects the glans and plays an important role in the mechanical function of the penis during sexual acts. [16][17][18][19][20] Recent studies, several of which were not included in the AAP report (although they were published within the inclusion period of 1995-2010), suggest that circumcision desensitizes the penis 21,22 and may lead to sexual problems in circumcised men and their partners. [23][24][25][26][27][28][29] In light of these uncertainties, physicians should heed the precautionary principle and not recommend circumcision for preventive reasons.…”
