Histamine intolerance: lack of reproducibility of single symptoms by oral provocation with histamine: A randomised, double-blind, placebo-controlled cross-over study

Abstract: Oral provocation with 75 mg of liquid histamine failed to reproduce histamine-associated single symptoms in many patients. One may suggest that histamine-intolerant subjects reacted with different organs on different occasions. As a consequence, reproducibility of single symptoms alone may not be appropriate to diagnose histamine-intolerance whereas a global symptom score could be more appropriate. The fact, that the intake of DAO capsules compared to placebo led to a statistically significant reduction of tot… Show more

“…12,27 Second, the severity of symptoms was not assessed by a severity standard, as recommended by Komericki et al by lack of reproducibility of single symptoms following oral provocation with histamine in a randomized, DBPC experimental setting. 28 Taken together, our results suggest that histamine intolerance should be considered in patients visiting allergy clinics. Serum DAO activity measurements and DAO supplementation represent important tools to consolidate the diagnosis, reduce the level of uncertainty of this condition, and provide a causal treatment for the patients' symptoms and signs.…”

“…It has been published that even in patients with overt histamine intolerance oral provocation was positive in only 50% of those tested [14]. Very recently a multicenter study on the nonreliability of blinded oral histamine provocation to confirm histamine intolerance has been published [16]. Obviously oral histamine provocation test thus cannot be considered a gold standard in HIT.…”

“…The so-called histamine intolerance is more likely a “complex of symptoms that can be attributed to histamine only in individual cases” than an isolated clinical picture exclusively triggered by ingested histamine [27]. That would explain why it is often not possible to reproduce symptoms following small quantities of histamine in the patient history [27, 28]. It is possible that symptoms only appear in the presence of concomitant factors (see above).…”

“…In this context, an alteration in small intestinal permeability as a prerequisite of symptom onset may be of relevance. However, the study conducted by Komericki and coworkers [27, 28] raises the question of whether symptoms can be affected by orally administered histamine at all. The intake of the histamine-degrading enzyme DAO was described to reduce symptoms irrespective of whether or not histamine was administered [28].…”
