Hip Arthroscopy to Remove Loose Bodies After Traumatic Dislocation

Abstract: Loose bodies are routinely present after closed treatment of hip dislocations or wall fractures not otherwise requiring surgery, even when radiographs are negative. Hip arthroscopy may be indicated for loose body removal when open treatment is not otherwise necessary.

“…If not identified and treated in a timely manner, incongruity can lead to secondary degenerative osteoarthritis. 2,3 Obstructions may include a complex composed of the posterior margin of the acetabulum, capsule and labrum, osteochondral loose body fragments, or ligamentum teres avulsions. [3][4][5][6] Incongruity may be subtle with plain radiography and small bony fragments and/or soft-tissue entrapment are often obscured and overlooked.…”

“…Radiographs in this case showed increased joint space after initial closed reduction. Mullis and Dahners 10 found that 7 of 9 hip dislocation patients had loose bodies although no loose bodies were detected preoperatively and the joints were congruent on imaging. This illustrates that conventional radiographs and CT scans do not depict the extent of chondral or labral pathology associated with traumatic hip dislocation.…”
