Highly Active Antiretroviral Therapy is Associated with Decreased Incidence of Sexually Transmitted Diseases in a Taiwanese HIV-Positive Population

Abstract: There are reports of increased sexual risk behaviors in the HIV-positive population since the introduction of highly active antiretroviral therapy (HAART). Little is known about the effects of the case management (CM) program and HAART on sexually transmitted diseases (STDs) in Taiwan. HIV-positive subjects, who visited the outpatient clinics of Taoyuan General Hospital between 2007 and 2010, were enrolled. A total of 574 subjects and 14,462 person-months were reviewed. Incident STDs occurred in 104 (18.1%) su… Show more

“…[ 11 , 28 ] The clinical role of ART on prevention of STIs other than HIV infection is pondered. Although ART has been associated with a lower incidence of STIs, including syphilis, [ 29 ] the association between ART and repeat syphilis in HIV-infected patients remained controversial. [ 10 , 19 , 30 32 ] Our national data demonstrated that retention in HIV care with ART adherence rate of ≥ 85% were associated with a lower risk of repeat syphilis.…”

“…[ 34 ] In Taiwan, HIV-infected patients could easily access to free ART since 1997, and since 2007 a national integrated case management program was launched to reduce risky behaviors in AIDS referral hospitals. [ 25 , 29 ] Other than antiviral agents, integration of program of assessment of ART adherence, health education and counseling of STIs prevention were provided by clinician and HIV case manager is important strategy for control of syphilis. [ 25 , 26 ] Previous studies have demonstrated that risky behaviors among HIV-infected patients will decrease when linked to and retained in primary care [ 35 ] and with clinician-delivered intervention.…”

“…4 There are dyadic benefits to early HAART initiation and adherence; viral suppression improves health outcomes and reduces the likelihood of transmission to an HIV-negative person. 6-12 Despite evidence indicating that male same-sex dyads may benefit from improved HAART adherence, no HIV interventions currently exist to address dyadic needs across the care continuum.…”

“…14,15 Patients on ART have not only a reduced risk of HIV transmission to their sero-discordant partners, 16,17 but also a reduced risk of acquiring other STDs. 18 A recent study has indicated that home-based interventions can significantly improve the adherence of ART patients. Sustaining a matching scale-up of ART and adherence interventions is necessary in reducing disease burden of HIV infection in the affected populations.…”
