Higher rate of central nervous system involvement by flow cytometry than morphology in acute lymphoblastic leukemia

Abstract: CSF FC detects CNS disease in ALL patients at diagnosis at a rate double than CP alone and is statistically associated with an elevated LDH level. It should be incorporated in the evaluation of CSF to detect CNS involvement.

“…In addition, mediastinal involvement is often seen in T-ALL/LBL. 4 At the time of diagnosis, around 6% of ALL patients show CNS involvement, 5 and if no CNS prophylaxis is given, then CNS disease is likely in 50 to 75% of patients. On the contrary, when CNS prophylaxis is instituted, CNS disease is likely in 2 to 10% of ALL patients, thus improving the long-term prognosis.…”

“…28 FCI, using at least six, but nowadays in some laboratories eight to 10 colors, has led to more specific and sensitive diagnostics. 24,29 This has allowed for improved detection of CNS involvement. The use of molecular tools (ie, polymerase chain reaction [PCR] and NGS) for low-level CSF involvement is still under study.…”

“…CNS involvement at the time of acute lymphoblastic leukemia diagnosis is noted in about 6% of patients [ 10 ]. The rate of detection of CNS disease is maximized by including cytological evaluation with flow cytometry [ 11 , 12 ].…”
