High Serum Vascular Endothelial Growth Factor Levels Predict Poor Prognosis after Radiofrequency Ablation of Hepatocellular Carcinoma: Importance of Tumor Biomarker in Ablative Therapies

Abstract: High pre-treatment serum VEGF levels predict poor prognosis after RFA of HCC. This study highlights the importance of tumor biomarker as a prognostic predictor in ablative therapy for HCC, which has an intrinsic problem of unavailability of histopathological prognostic features.

“…In total, there were four tissue OS studies (Chow et al, 1997;Jeng et al, 2004a;Deli et al, 2005;Sheen et al, 2005), six tissue DFS studies (Jeng et al, 2004a;Sheen et al, 2005;Guo et al, 2006;Wada et al, 2006;Zhang et al, 2006;Ho et al, 2007), seven serum OS studies (Poon et al, 2001(Poon et al, , 2004a(Poon et al, , b, 2007Chao et al, 2003;Kim et al, 2004;Jeng et al, 2004b;Treiber et al, 2006), and five serum DFS studies (Poon et al, 2001(Poon et al, , 2007Chao et al, 2003;Jeng et al, 2004b;Treiber et al, 2006). “…Similar findings were reported in a recent study of patients who underwent radiofrequency ablation. 70 The rise in VEGF levels after local therapy for patients with HCC indicates a potential for biomarkerguided antiangiogenic therapy in this setting. Furthermore, circulating VEGF levels may be predictive of the outcomes after local therapies or a biomarker-guided adjunctive antiangiogenic approach.…”

“…Two studies using the endothelial marker CD34 to identify neovascularisation have reported that high microvessel density was a significant predictor to poor disease-free survival after resection Wada et al, 2006;Poon et al, 2007). The mechanisms responsible for angiogenesis in HCC are not completely understood.…”

“…Although well-differentiated HCC express higher levels of VEGF, it has also been shown that circulating VEGF can be detected in patients with more undifferentiated carcinomas (Torimura et al, 1998). Moreover, a correlation between VEGF expression and survival has been described (Poon et al, 2007). Other factors important for neoangiogenesis in HCC include fibroblast growth factor 2 (FGF2), angiogenin and angiopoietin.…”
