High-Resolution Computed Tomography and Lung Ultrasound in Patients with Systemic Sclerosis: Which One to Choose?

Abstract: Imaging plays a pivotal role in systemic sclerosis for both diagnosis management of pulmonary complications, and high-resolution computed tomography (HRCT) is the most sensitive technique for the evaluation of systemic sclerosis-associated interstitial lung disease (SSc-ILD). Indeed, several studies have demonstrated that HRCT helps radiologists and clinicians to make a correct diagnosis on the basis of recognised typical patterns for SSc-ILD. Most SSc patients affected by ILD have a non-specific interstitial … Show more

“…As we known, surgical lung biopsy (SLB) was not recommended in the patients having HRCT images presenting a typical UIP pattern, because of the relatively high complication rates and mortality in this part of patients ( 12 ). Compared to SLB, TBCB had lower morbidity and it was suggested that sometimes TBCB could be proposed in patients with a typical radiological UIP pattern ( 7 ).…”

“…Fibrotic pattern included the following HRCT features ( 11 , 12 ): honeycombing or reticulation with traction bronchiectasis (UIP or probable UIP), reticulation predominant (indeterminate for UIP), and ground-glass opacities with traction bronchiectasis or honeycombing (others).…”

“…Cardiac features of PH are represented by a right ventricle hypertrophy, defined as a wall thickening of more than 4 mm or leftward bowing of the interventricular septum, and right ventricular dilatation, defined as a right ventricle to left ventricle diameter ratio of more than 1:1 at the midventricular level on axial image. “…The use of LUS for the quantification of the extent of ILD has been partially explored in previous studies [ 20 ]. This was the case in the visual ILD extent scoring method of Warrick et al, with significant correlations between the increasing number of B-lines and the increasing extent/score [ 21 , 22 ].…”
