Background Type 2 diabetes mellitus (T2DM) in adolescents represents a clinical challenge related to lifestyle and obesity; however, only a few data are available in developing countries. Therefore, our aim was to investigate the prevalence of T2DM and prediabetes among Brazilian adolescents, as well as to describe the cardio‐metabolic profile according to the diagnosis. Methods This is a cross‐sectional school‐based multicenter study including youth aged 12 to 17 years from cities with more than 100 000 inhabitants in Brazil (n = 37 854 students). Fasting glucose, hemoglobin A1c (HbA1c) and other cardio‐metabolic risk factors were measured. Prediabetes was defined by glucose levels 100 to 125 mg/dL or HbA1c 5.7% to 6.4%. T2DM was defined by self‐report, glucose ≥126 mg/dL or HbA1c ≥ 6.5%. Multinomial logistic regression was used to estimate the odds ratio (OR) of prediabetes or T2DM according to covariates. Results Prevalences of prediabetes and T2DM were 22.0% (95% confidence interval [CI] 20.6%‐23.4%) and 3.3% (95% CI 2.9%‐3.7%), respectively. This estimates represented 213 830 adolescents living with T2DM and 1.46 million adolescents with prediabetes in Brazil. Prevalences of cardio‐metabolic risk factors were higher in adolescents with prediabetes and T2DM. In the multinomial logistic model, obesity (OR 1.59, 95% CI 1.20‐2.11), high waist circumference (OR 1.51, 95% CI 1.13‐2.01), and skipping breakfast (OR 1.48, 95% CI 1.21‐1.81) were associated with an increased OR for T2DM, while studying at rural area (OR 0.56, 95% CI 0.41‐0.78) was associated with a decreased OR for T2DM. Conclusions The prevalence of T2DM and prediabetes was high among Brazilian adolescents, which highlights that this disease became a public health challenge not only among adults in Brazil.

Characterization of health issues in young first‐time blood donors

Background and objectives Blood donors, especially young donors, are considered a healthy segment of the population. We sought to identify medical issues that may warrant medical referral in young first‐time blood donors. Materials and methods A retrospective cohort study was performed in first‐time donors ages 16–22 who presented in a system of nineteen regional United States blood centres over 10 years. Donor health attributes characterized include body mass index, blood pressure, total cholesterol and pre‐donation haemoglobin. Using standardized definitions, overweight and obese body mass, hypertension, elevated cholesterol and anaemia were identified and characterized in this donor population. Results Among 825 041 young first‐time donors presenting between January 2009 and December 2018, with available measurements, 46·9% were either overweight or obese, 59·8% demonstrated high blood pressure (22·2% elevated blood pressure, 37·6% stage 1 or 2 hypertension), elevated cholesterol was identified among 6·3% of males and 8·8% of females, and anaemia was present in 3·5% of males and 5·2% of females. During the study period, all unfavourable health outcomes significantly increased in prevalence (P < 0·0001) when comparing 2009 vs. 2018 rates. Conclusion Elevated weight and obesity are common in young first‐time allogeneic United States blood donors, with fewer donors having elevated total cholesterol or anaemia. Such medical issues may have significant importance for future health and well‐being as well as continued donor eligibility. Blood centres may be able to help support the identification and mitigation of important medical issues in donors and provide a public health benefit.

Background and Objectives Previously it has been shown that platelet (PLT) storage performance is consistent by donor. Differences involved metabolic activity, which might be caused by mitochondrial (dys)function, associated with age and age‐related diseases like Type 2 diabetes (T2D). We aimed to test PLTs from young donors in comparison with PLTs from older donors with or without diagnosis for T2D. Materials and methods Fifteen whole blood donors <30 year were selected, and single‐donor platelet concentrates (sPC) were prepared from buffy coats (BC) and plasma. Also, 2 × 11 sPC were prepared from matched donors >45 years with and without T2D. The sPC were stored for 8 days and analysed at regular intervals for in vitro quality. Results Donors were 24 ± 3, 60 ± 7 (without T2D) and 59 ± 8 (with T2D) years old. All sPC groups had comparable volume and PLT content. On Day 8, sPC from young donors showed higher pH37°C than sPC from older donors (6.84 ± 0.15 vs. 6.40 ± 0.48, P < 0.01), due to lower lactate production. Also, CD62P expression (22.9 ± 7.4 vs. 48.8 ± 24.0%, P < 0.01) and HSR reflected better in vitro quality. PLT storage properties of sPC obtained from T2D donors (pH = 6.51 ± 0.35) were not different from sPC of matched donors (pH = 6.40 ± 0.48). No differences in mitochondrial membrane potential were detected between the groups. Conclusion Platelets from young donors exhibited the best storage conditions. On average, PLTs from older donors showed poorer in vitro quality but, considering the sub‐optimal storage conditions, the implications for the daily blood bank routine is probably small. No association with T2D was found.
