High-intensity stretch treatment for severe postoperative adhesive capsulitis of the shoulder

Abstract: Patients with postoperative adhesive capsulitis of the shoulder who are unable to reach their PT treatment goals with a standard protocol of PT may benefit from the addition of HIS to their treatment regimen. HIS could be a valuable adjunct to PT for treating postoperative adhesive capsulitis in appropriate patients.

“…This highintensity stretch is controlled by the patient, is focused at the end-range of motion of the joint, and can permanently elongate scar tissue after it has already formed inside the shoulder. This HIS device has been previously shown to successfully treat motion loss in the shoulder [12,13].…”

“…In one study, treatment outcomes (forward flexion and combined internal/external rotation) in 60 patients with postoperative adhesive capsulitis were compared between two groups: (1) patients who used the HIS device after reaching a plateau in their recovery following traditional physical therapy (PT + HIS) and (2) patients who showed no plateau in their recovery with physical therapy alone (PT only) [18]. Patients in the PT + HIS group had significantly worse initial elevation and combined rotation when compared to PT-only patients.…”

“…Adhesive capsulitis is the leading cause of pain at the shoulder joint in middle aged and elderly persons [5]. The exact underlying cause for adhesive capsulitis is unknown, but the non-dominant hand and females are considered to be more affected [6]. It is also more prevalent in the 40-70 years age group [2,5] and it is estimated that 2-3% of the general population is affected with this pathology [7].…”
