High-intensity meropenem combinations with polymyxin B: new strategies to overcome carbapenem resistance inAcinetobacter baumannii

Abstract: Intensified meropenem dosing in combination with polymyxin B may offer a unique strategy to kill CRAB irrespective of the meropenem MIC.

“…Polymyxins have remained effective against Acinetobacter infections (13). Available approaches use novel combinations of drugs, such as daptomycin-colistin-teicoplanin and intensified meropenem-polymyxin B (21). The presence of mcr-4.3 and its location on a plasmid would facilitate the spread of mcr-4.3 among Acinetobacter strains.…”

“…However, of the few studies examining meropenem against a variety of pathogens in the HFIM, none have included a half-life representative of ARC (31)(32)(33). In addition, very few have examined the emergence of resistance to meropenem, which is known to occur primarily via downregulation of the gene for the outer membrane porin OprD (which allows entry of carbapenems into the cell) and overexpression of efflux pumps MexAB-OprM, MexXY-OprM, and MexCD-OprJ (34-40).…”

“…In addition, resistance to both polymyxins has increasingly been reported, and in vitro studies have shown that the emergence of resistance is relatively common even by isolates exposed to polymyxin concentrations similar to those expected in vivo after the administration of an adequate dose of polymyxins (11)(12)(13). To counteract this occurrence, in vitro exposures to very high concentrations of PMB through the use of distinct strategies, such as administration of a loading dose or the use of high doses in combination schemes, have been evaluated, with promising results (14)(15)(16)(17). However, the toxicity of such regimens, in particular, the toxicity related to the infusion of high doses, is largely unknown.…”
