High Incidence of Lymph Node Metastasis in Major Salivary Gland Cancer

Abstract: We found a high incidence of lymph node metastasis from major salivary gland cancers. Neck dissections should be considered as an integral part of the surgical approach in patients with major salivary gland cancer, especially if no postoperative radiation therapy is planned.

“…Stennert et al found a high incidence of occult metastatic disease in the lymph nodes of the deep lobe and recommended a complete parotidectomy in every case of primary parotid gland carcinomas. 8 In a study with 128 mucoepidermoid carcinomas, most recurrences were detected in the parotid bed, pointing to the need for adequate control, at least at the site of the primary tumor. 12 In our experience, however, a malignant tumor of the parotid gland does not necessarily have any clinical (pain, facial palsy) or imaging signs of malignancy (enlarged neck nodes, blurred lesion margins, elastographic ''garland'' sign 13 ).…”

“…1,18,29,31 Myoepithelial carcinomas may be characterised by nodal spread and these may be addressed surgically as well. 32 Chemotherapy has no effectiveness against these tumours. 1,18,28,29,31 The tumour in this case was well circumscribed, had a soft tissue density, and was partially surrounded by reactive bone.…”
