High Incidence of Herpes Zoster in Patients with AIDS Soon After Therapy with Protease Inhibitors

Abstract: A high incidence of herpes zoster was noticed among patients with AIDS, shortly after addition of a protease inhibitor to their baseline treatment with nucleoside analogue reverse-transcriptase inhibitors. Within a median follow-up of 64 weeks (range, 34-103 weeks), 14 patients (7%) had a first episode or a recurrence of herpes zoster (6.2 episodes per 100 patient-years). No episodes of zoster were diagnosed before week 4. Twelve episodes (86%) occurred between weeks 4 and 16. The risk of zoster was independen… Show more

“…Cohort studies of adults and children indicated that the incidence of VZV disease ranged from 5 to 12%, with most cases occurring in the first 16 weeks of therapy (Table 2). A low CD4 T-cell count was a risk factor in children (140) but not in adults (40,42,90). VZV disease after commencement of ART usually presents as shingles affecting a single dermatome but may present as multidermatomal shingles, Ramsay-Hunt syndrome (M. A. French, personal observations), or myelitis (20,29).…”

“…Following the introduction of ART in the mid-1990s, cases of dermatomal herpes zoster were up to five times more common in patients who had recently commenced ART (1,90). Cohort studies of adults and children indicated that the incidence of VZV disease ranged from 5 to 12%, with most cases occurring in the first 16 weeks of therapy (Table 2).…”

“…Also, HIV-infected patients initiating both therapies are more prone to develop transient chest radiographic worsening than HIV-uninfected and HIV-infected patients not on AR therapy 4 . Enhancing host immunity following initiation of AR therapy or its intensification with a protease inhibitor has also been implicated in the emergence of cytomegalovirus retinitis 6 , Mycobacterium avium complex lymphadenitis 14 , development of zoster rash 9 , rapid worsening of Kaposi's sarcoma lesions 7 and development of multiple cerebral lesions compatible with progressive multifocal leukoencephalopathy in AIDS patients 7 .…”
