High-Grade Prostatic Intraepithelial Neoplasia

Abstract: High-grade prostatic intraepithelial neoplasia (HGPIN) has been established as a precursor to prostatic adenocarcinoma. HGPIN shares many morphological, genetic, and molecular signatures with prostate cancer. Its predictive value for the development of future adenocarcinoma during the prostate-specific antigen screening era has decreased, mostly owing to the increase in prostate biopsy cores. Nevertheless, a literature review supports that large-volume HGPIN and multiple cores of involvement at the initial bio… Show more

“…However, there was a great range of miR-125b levels in BPH cells as evident by the large standard deviation. PIN is considered to be a precursor for the development of prostate carcinoma [ 34 , 35 ]. Many of the genetic changes that drive prostate cancer occur early in pathogenesis and are evident in PIN cells.…”

“…Discovery of hgpin comes by invasive extraction of a biopsy sample taken because of suspicion for pca 4 , making the experience of the biopsied men much more similar to that of preoperative pca patients than to that of men with elevated psa and no biopsy. Given the semantic association between "neoplasm" and "tumour" (as occurs with dcis 41 ), hgpin can also carry the connotation of cancer.…”

“…Approximately 115,000 cases of hgpin are diagnosed each year in North America 6,7 . Diagnosis of hgpin has been found to be highly predictive of pca 4,8 . Approximately one third of men who test negative for cancer but positive for hgpin upon prostate biopsy eventually develop prostate adenocarcinoma 8,9 .…”

“…In terms of biological variability, the study does not demonstrate performance on a common diagnostic entity-high grade prostatic intraepithelial neoplasia (HGPIN). However, HGPIN represents 4-16% of prostate needle biopsies and can mimic the appearance of prostate cancer [40][41][42] , and therefore could be expected to reduce the specificity of such a system. The work of Campanella et al additionally was unprecedented in terms of image variability covered-it includes a large dataset encompassing slides prepped in 44 countries, representing a degree of variability in staining and tissue preparation characteristics which previous studies have lacked.…”
