High Grade Leiomyosarcoma of the Testes

Abstract: Testicular leiomyosarcoma is a rare tumor. It may arise secondarily following exposure to radiotherapy, chronic inflammation, or usage of high dose anabolic steroids. However, in absence of risk factors, it is rarely seen. Only 15 cases of Primary Intra testicular leiomyosarcoma have been reported in world literature. We present a case of testicular tumor in an elderly male. Preoperative work up showed raised Lactate Dehydrogenase (LDH) levels. He underwent high orchidectomy. Histopathology and immunohistochem… Show more

“…Primary leiomyosarcoma in testis was an unusual finding with only some reported cases in published articles (5). Said that, to date, only 15 patient with primary leiomyosarcoma in testis have been reported (3). Reported cases with primary leiomyosarcoma in testis had the average age of 50 years which is compatible with our case.…”

“…Leiomyosarcoma in testis is an uncommon tumor which might happen after radiotherapy, chronic inflammation, or anabolic steroids use for long time. However, in absent of risk factors, it is seldom found (3). Here we report a patient with primary leiomyosarcoma in testis, which was thought to derive from normal testis organ include smooth muscle cells, such as blood vessels or seminiferous tubules' contractile cells.…”

“…It seems to be not different from other malignant testicular tumors. Tumor markers (AFP, β-hCG and LDH) used in the previously reported cases were almost within the normal range, except two cases: one case with an elevated LDH [14] and the second with elevated β-hCG [16]. This case had an elevated LDH.…”

“…Although, adjuvant treatment chemotherapy and radiotherapy may be used for intratesticular leiomyosarcoma with high stage [14]. This case was treated with HIO without any adjuvant treatment and after 24 months of follow-up the patient was free of disease.…”

“…We cannot deny the incidental concomitance of the diseases. However, chronic inflammation is suggested to be a risk factor for testicular LMS in some case reports ( 8 - 11 ), and further study may thus be needed to reveal the causal relationship between LMS and chronic bowel inflammation.…”
