High-grade glioma in elderly patients: can the oncogeriatrician help?

Abstract: Gliomas are the most frequent primary brain tumors in adults. As the population ages in Western countries, the number of people being diagnosed with glioblastoma is expected to increase. Clinical management of elderly patients with primary brain tumors is difficult, owing to multiple comorbidities, polypharmacy, decreased tolerance to chemotherapy, and an increased risk of radiation-induced neurotoxicity. A few specific randomized studies have shown a benefit for radiotherapy in elderly patients with good perf… Show more

“…In this study, we found that the radiation-induced hippocampal vascular injury and the subsequent neurocognitive decline are age- and sex- dependent. Although the exact mechanism of the age-dependent radiosensitivity remained unknown, previous studies 24, 25 have shown that RT has more adverse effects on older patients. Moreover, the radiation-induced aging-like pathology in cerebrocortical cells has been suggested 26 , including shrinkage of cortical thickness, activation of cell-cycle arrest pathway and inhibition of DNA double strand break repair.…”

“…After surgery, the standard treatment of gliomas includes chemotherapy combined with or without radiotherapy after the radical resection. Supportive care and treatment with corticosteroids and antiepileptics (AEs) play an important role in elderly patients [ 3 ].…”

“…Hence, the better outcome in radical patients may be related to the selection of better prognosis patients. Moving forward, the involvement of a oncogeriatrican and standardized evaluation tools should also be incorporated into MDT to help guide treatment decisions …”

“…Moving forward, the involvement of a oncogeriatrican and standardized evaluation tools should also be incorporated into MDT to help guide treatment decisions. 22,23 Quality of life, which is especially important in GBM, was not always available from the notes and hence not evaluated in this study. Neurotoxicity and side effects of RT were not evaluated.…”
