High global consumption of potentially inappropriate fixed dose combination antibiotics: Analysis of data from 75 countries

Abstract: Antibiotic fixed dose combinations (FDCs) can have clinical advantages such as improving effectiveness and adherence to therapy. However, high use of potentially inappropriate FDCs has been reported, with implications for antimicrobial resistance (AMR) and toxicity. We used a pharmaceutical database, IQVIA-Multinational Integrated Data Analysis System (IQVIA-MIDAS®), to estimate sales of antibiotic FDCs from 75 countries in 2015. Antibiotic consumption was estimated using standard units (SU), defined by IQVIA … Show more

“…Ampicillin-cloxacillin is listed as “not recommended” in the WHO AWaRe antibiotic classification database. It is considered as an inappropriate fixed-dose combination and is a problem not only in Uganda but in many low- and middle-income countries [ 64 , 65 ]. Ampicillin-cloxacillin was removed from the 2016 Uganda Clinical Guidelines.…”

“…These disparities are partly due to existing healthcare and diagnostic facilities, affordability, and antibiotic availability in those countries. 40,41]. Importantly, antibiotic FDCs has been reported about the lack of proven efficacy, increasing toxicity, potential for developing AMR.…”

“…It is well known fact that India is a global paradise of pharmaceuticals and has the highest numbers of FDC antibiotics[40,41]. Conversely, FDC use is potentially inappropriate and majority of the FDC antibiotics were not approved by either US FDA or WHO EML[18,…”

“…In this context, Bortone et al [11] reported an increase in antimicrobial consumption of 65% between 2000 and 2015. Recently, the pandemic due to COVID-19 has dramatically favored the large-scale use of a group of antimicrobials for therapeutic purposes (azithromycin, ivermectin, remdesivir, favipiravir, among others) [12].…”
