High efavirenz levels but not neurofilament light plasma levels are associated with poor neurocognitive functioning in asymptomatic HIV patients

Abstract: The aim of this study is to assess the effect of efavirenz exposure on neurocognitive functioning and investigate plasma neurofilament light (Nfl) as a biomarker for neurocognitive damage. Sub-analysis of the ESCAPE-study, a randomised controlled trial where virologically suppressed, cognitively asymptomatic HIV patients were randomised (2:1) to switch to rilpivirine or continue on efavirenz. At baseline and week 12, patients underwent an extensive neuropsychological assessment (NPA), and serum efavirenz conce… Show more

“…First, cognitive function was not assessed in our study. The impact of EFV on cognitive function remains controversial, as several studies show a deterioration of cognitive profile related to EFV use, [ 5 , 33 ] while others do not present any improvement in cognitive performances after EFV withdrawal. [ 22 , 34 ] The evaluation of the effect of EFV switch off on neurocognitive disorder with a larger sample size and longer follow-up time should be performed in the future.…”

“…A further study of 89 hemodialysis patients found that 63 (71%) were poor sleepers. The question is whether the prevalence of insomnia in people with HIV is comparable to that in the general population or in people with other chronic diseases [15,16] Although sleep disturbances are frequently reported in people with HIV and AIDS, only a few studies have systematically evaluated their prevalence. Only one study identified using the PSQI to classify as poor sleepers.…”

“…Moreover, the majority of these studies involve HIV+ participants who are virally-suppressed and receiving cART [ 181 , 217 ]. Only a few - those primarily recruiting participants in the Global South - include ART-naïve participants [ 189 ].…”
