High-Dose Porcine Hematopoietic Cell Transplantation Combined With Cd40 Ligand Blockade in Baboons Prevents an Induced Anti-Pig Humoral Response

Abstract: We believe these studies are the first to consistently demonstrate prevention of a secondary humoral response after cell or organ transplantation in a pig-to-primate model. The development of sensitization to the murine elements of the anti-CD40L monoclonal antibodies suggests that nonresponsiveness to cell membrane-bound antigen (e.g., alphaGal) is a specific phenomenon and not a general manifestation of immunological unresponsiveness. T cell costimulatory blockade may facilitate induction of mixed hematopoie… Show more

“…These results also have implications for xenotransplantation, because sensitization to ␣Gal following exposure to pig tissues could be overcome by blocking the interaction of CD40 and CD154. Consistent with this notion, it has been shown in baboons that preconditioning with a combination of whole body and thymic irradiation, extracorporeal immunoadsorption of ␣Gal Ab, mycophenolate mofetil, antithymocyte globulin, and anti-CD154 leads to humoral hyporesponsiveness following infusion of cytokine-mobilized pig blood cells (32). Indeed, the observation that ␣Gal-specific Ab production is T cell dependent suggests that T cell immunosuppression will be a major part of host preparation for xenotransplantation.…”

“…It is clear that two different populations of B cells contribute to the anti-Gal Ab response. The first is a Tindependent B-1B cell subset that is responsible for T-independent anti-Gal IgM production (32), and the second is a conventional T-dependent B-2 population that produces anti-Gal IgG (17,(33)(34)(35). It has recently been reported that the signaling thresholds for B-1 and B-2 cells differ, and differentiation of B-0 and B-2 cells to B-1 cells that have increased activation thresholds can result in functional B cell tolerance (36 -38).…”

“…The R/S ratio represents the numbers of replacement/silent mutations as previously described (20). The relative differences in affinity between hybridomas (listed in affinity measurements and suggest that upon affinity maturation some anti-Gal IgG from Gal Ϫ/Ϫ mice can attain affinity comparable to that of humans or primates.…”
