High‐dose intravenous immunoglobulin therapy in neonatal immune haemolytic jaundice

Abstract: A controlled study was conducted to assess the role of high‐dose i.v. immunoglobulin (HDIVIG) therapy in neonatal immune haemolytic jaundice. Patients with ABO and/or Rh incompatibilities proved by significant hyperbilirubinaemia (<204mmol 1−1), positive direct antiglobulin test and high reticulocyte count (>10%) were randomly assigned to receive either conventional phototherapy alone or phototherapy with high‐dose i.v. immunoglobulin (1 g kg−1, over 4h) as soon as the diagnosis was established. Exchange trans… Show more

“…The use of IVIG was first shown to be effective for treating blood group incompatibility in the 1990's ( 11 ). Since then, it has been used to treat HDN to avoid exchange transfusion, but there is still no consensus on its use.…”

“…The need for exchange transfusion treatment after IVIG use has varied among studies. Many factors, including age upon admission to the hospital, duration and strength of phototherapy, and dose and time of administration of IVIG may influence the response to IVIG ( 11 , 17 , 21 ). Hammerman et al reported that infants who did not respond sufficiently to IVIG therapy have higher carboxy-hemoglobin levels, which means that their hemolysis was more severe ( 22 ).…”

“…Usage of IVIg has been found to decrease the need of ET in both Rh and ABO isoimmune HDN. [1][2][3][4] High dose of IVIg (0.5-1 g/kg) is recommended by the American Academy of Pediatrics (AAP) in 2004 as an additional treatment of isoimmune HDN. 5 Since then, IVIg has been successfully utilized in isoimmune HDN.…”

“…The administration of intravenous immunoglobulin (IVIG) is an adjunctive treatment by blocking of the reticuloendothelial Fc receptor sites and thereby preventing the extravascular destruction of neonatal red blood cells (RBCs) sensitized by maternal antibodies [7, 8]. It was shown to decrease the need for blood exchange transfusion [9, 10].…”

“…More recently, an association with necrotizing enterocolitis has also been described [13]. However, IVIG has been deemed safe, and is generally well tolerated with a limited side effect profile [7-10, 14, 15].…”
