High‐dose intravenous immunoglobulin for post‐transfusion purpura

Abstract: Five patients with post-transfusion purpura (four due to Zw(a), one presumably due to HLA antibodies) were treated with intravenous immunoglobulin (IgG) at doses of 0.4 g per kg body weight. IgG therapy was immediately effective as indicated by cessation of bleeding and rise of platelet counts in four out of five cases.

“…Recent case reports have shown the value of high-dose intravenous IgG in producing a prompt platelet response in PTP [26,33,34]. In the present case, bleeding stopped and there was a gradual increase in the platelet count after intravenous IgG, reaching normal levels after 7 days.…”

“…The best therapy for PTP, prompting recovery of the platelet count within several days, is IVIg [21][22][23][24]26,27]. Plasmapheresis is also effective [16,[28][29][30][31][32][33][34][35].…”

“…Case reports suggest that therapy with plasmapheresis and IVIg shorten the duration of thrombocytopenia [13,[21][22][23][24][25][26][27]. IVIg is considered first-line therapy, with one or two doses (0.4 mg/kg) resulting in improved platelet counts within a few days [21,23,24,26]. Plasmapheresis is an important component of therapy Contract grant sponsor: NIH/NCI; Contract grant number: T32-CA-009679; Contract grant sponsor: NIH/NHLBI; Contract grant number: HL-54500 [16,[28][29][30][31][32][33][34][35], but it may be second to IVIg because of its relative unavailability and the risk of necessitating an invasive procedure [34].…”

“…It has been postulated that the more long term effect is due to reduction in platelets antibody production [12]. In PTP, the majority of cases respond rapidly to IVIG [13]. The exact The patient, a 48-year-old multiparous woman was admitted with fresh rectal bleeding and a widespread purpuric rash.…”
