High-dimensional proteomics identifies organ injury patterns associated with outcomes in human trauma

Abstract: @ShimenaL; @BrachHerzig; @JunruWu1; @SultanAbdelH; @SciosciaPio; @JBonarotiMD; @macky_neal; @jishnu1729; @Jjasonsperrymd; @TheBilliarLab; @trbilliar; #universityofpittsburghUsing comprehensive plasma proteomics, we identified that biomarkers for liver, pulmonary, neurologic, and particularly cardiac injury remained elevated in patients with complex clinical courses after severe injury.

“…Similar studies have been performed with burn injured patients [106]. Metabolomic studies using mass spectroscopy on venous blood from bluntly injured patients within 24 hours can provide essential clues as to the development of chronic critical illness [107]. Differences in aromatic amino acid metabolism along with metabolites associated with oxidative stress were evident.…”

“…12,13 Posttraumatic multiple organ dysfunction is generally held to result from a dysregulated immune response to injury, and the precise mechanisms and pathways involved have been subject to intense scrutiny. [14][15][16][17] Synchronized activation of coagulation and inflammation, a process termed thromboinflammation, has surged to prominence as a result of the COVID pandemic and appears central to the pathophysiology of organ failure in this condition. [18][19][20] Evidence for this phenomenon as a mechanism of organ failure in trauma patients is comparatively sparse, but patients with TIC have significantly higher rates of multiple organ dysfunction, 21 and aggressive treatment of coagulopathy is associated with a reduction in the incidence and severity of organ failure.…”

“…Of note, four DAMP proteins were found to have elevated concentrations in the endotheliopathy of trauma group, which have been shown to mediate a multitude of endothelial impairment and permeability, inflammatory responses, and end-organ injury (128). Li et al (129) conducted a secondary analysis of trauma patients included in the Prehospital Air Medical Plasma and Study of Tranexamic Acid During Air and Ground Medical Prehospital Transport trials using high-dimensional proteomics. The study demonstrated that SDC-1 was associated with all eight cardiac biomarkers (129), which could be a possible mechanism for the development of MOD.…”

The study demonstrated that SDC-1 was associated with all eight cardiac biomarkers (129), which could be a possible mechanism for the development of MOD. While both studies included TBI patients, neither was specifically powered to evaluate the TBI cohort.…”
