Heterotopic gastric cyst of the oral cavity

Abstract: The heterotopic gastric cyst of the oral cavity is a rare lesion. A recurrent gastric cyst in the floor of the mouth of a young female patient is presented. Current theories dealing with the histogenesis of these lesions are presented and discussed. Based on the available embryologic and clinical information, a new histogenetic concept for the development of the heterotopic gastric cyst is proposed.

“…One theory states that heterotopic mucosa may be derived from entrapped embryonic gastrointestinal epithelium or ectoderm from the primitive stomodeum [8]. A second theory suggests that endodermal cell rests of the stomodeum are trapped by the lateral lingual swellings and subjected to inductive influences that cause their differentiation into gastrointestinal epithelia [13,14]. The latest and most current theory supports the idea that a disturbance in the development of the notochord and surrounding structures accounts for misplaced segments of gastrointestinal mucosa in which adherent endodermal cells are caught during the infolding of the notochordal plate [15].…”

“…Another theory, developed by Daley et al 5 in 1984, proposed that endodermal cell rests of the stomodeum became trapped by the lateral lingual swellings and were subject to inductive influences causing differentiation into gastrointestinal epithelia. In 1988, Woolgar and Smith 6 performed a mucin histochemical study of an enteric duplication cyst of the tongue and found well-differentiated columnar and goblet cells.…”

“…It has also been suggested that the entrapped endoderm may arise from the lining of the primitive stomatodeum. 8,22 Irrespective of the pathogenesis of heterotopic cysts, the lesions are benign and treated by surgical excision. Our experience with the present case confirms the importance of obtaining a computed tomographic and/or magnetic resonance scan in the preoperative evaluation.…”
