Hereditary motor–sensory neuropathy and movement disorders

Abstract: To explore the relationship between hereditary motor and sensory neuropathy (HMSN) and movement disorders, we examined 7 patients with HMSN referred to our Movement Disorders Clinic and surveyed members of the Charcot-Marie-Tooth association. The following movement disorders were observed in the index patients: postural tremor in 6, rest tremor in 3, and Parkinsonism and dystonia in 2. Tremor, present in 40% of the 201 patients who responded to the survey, was first noted at a mean age of 36 years, and mostly … Show more

“…Given that hereditary neuropathies are considered to be clinically heterogeneous [1], polyneuropathy associated with tremor could be considered a CMT variant with central nervous system involvement in our patients [19]. In our opinion, there are 2 strong points that support this consideration.…”

“…One of the hypotheses is the presence of distorted peripheral inputs that mislead a central processor into producing tremor [7,18]. Nevertheless, not all patients with neuropathy develop tremor, and the occurrence of the tremor does not correlate with the severity of the neuropathy [19]. Some reports describe some degree of cerebellar dysfunction in neuropathic tremor similar to those described in essential tremor [20].…”

“…[87][88][89][90][91] Usually it is a mild postural tremor. Some cases, however, typically with chronic demyelinating peripheral neuropathies or "benign" monoclonal gammopathies ,suffer from severe and most disabling tremors.…”

“…While the characteristics and prevalence of tremor in patients with chronic inflammatory demyelinating polyneuropathy (CIDP) and hereditary motor-sensory neuropathies (HMSN) have been documented, 1,5 how often tremor occurs in patients with other acquired neuropathies having demyelinating features remains unclear, as does the nature of the relationship between the symptoms of neuropathy, tremor, and associated immunologic disorders. Little has been written on these topics, and virtually no in-depth physiologic studies have been performed on groups of patients with both peripheral neuropathy (PN) and tremor.…”
