Hereditary anterior megalophthalmos with posterior vitreoretinopathy: a surgical challenge

Abstract: SUMMARYAnterior megalophthalmos is an inherited condition characterised by enlargement of the anterior segment with associated glaucoma and vitreoretinopathy. These associations make surgical management very challenging. A 12-year-old boy, diagnosed and operated elsewhere for congenital glaucoma in both eyes, presented with raised intraocular pressure. Investigations revealed enlarged corneal diameter, increased anterior chamber depth with thick, clear cornea and no Haab striae while fundus evaluation revealed… Show more

“…Even in three eyes of our series with unremarkable macula, the best corrected visual acuity was not >20/80 perhaps due to ammetropic amblyopia. The age of presentation was lower in our cases as compared to previous literature [3,7,11], perhaps due to a greater referral of paediatric vitreoretinal cases to our tertiary care referral centre.…”

“…Enough literature exists on morphological types of cataract, cataract surgery and its complications in adult patients with AM [5]. The posterior segment changes in the form of vitreo-retinopathy and pigmentary glaucoma are scarcely reported in AM [1,3,6,7]. Scheimpflug imaging (SI) in a couple of cases of AM has also been reported [8,9].…”

“…It must be noted that glaucoma has been reported in anterior megalophthalmos - but those are largely secondary to lens subluxation or ciliary body abnormalities. Other reported ocular associations include iris hypoplasia, zonular weakness, angle abnormalities, Descemet’s detachment, posterior dislocation of lens, vitreous degeneration, and retinal detachment [ 8 , 9 ]. Cataract surgery in adults with anterior megalophthalmos has also been reported adequately in the literature [ 10 ].…”

“…Rao and Videkar reported the case of a 12-year-old boy with congenital glaucoma who was found to have wide lattice degeneration in the upper region of both eyes, which resulted in the formation of a tear and, ultimately, bullous RD. [12]…”
