Hereditary angio-oedema as a rare cause of small-bowel obstruction

Abstract: A 52-year-old man with known hereditary angio-oedema (HAE) presented with a 2-day history of progressive severe abdominal pain, distension, nausea, vomiting and constipation. CT of his abdomen and pelvis showed small-bowel obstruction and ascites. HAE is a rare autosomal dominant disorder caused by a C1 esterase deficiency and involves episodic oedema of subcutaneous and mucosal tissues. It commonly affects the face and limbs, causing deformity; the respiratory tract, causing life-threatening laryngeal swellin… Show more

“…In a review of 153 patients and 33,671 episodes of gastrointestinal attacks, less frequent symptoms were reported, including hemorrhagic diarrhea, tetany, and a single documented case of bowel intussusception [ 8 ]. There have been similar reports of small bowel obstruction and intussusceptions that resolved after infusion of C1-INH concentrate, but few have reported a bowel with impending ischemia [ 9 , 10 ]. In this case, the surgical procedure revealed strangulation of the distal jejunum to ileum incarcerated as an internal hernia due to adhesions forming a hernial band.…”

“…Evidence of the HAE-GI symptom overlap comes from both observational 57 and single-case studies 27,38,42,50,[67][68][69][70] (Table 2). HAE should be suspected when a patient presents with a history of recurrent cutaneous angioedema attacks.…”

“…Generally, clinical symptoms are not specific and may be ignored by patients as they are self-limiting (1). HA rarely mimics an acute abdomen, leading to unnecessary diagnostic laparotomy (3). Abdominal CT reveals the presence of peri-intestinal free fluid, bowel wall edema, and the "bowel target sign" and differentiates HA from other acute abdominal causes.…”
