Introduction: HER-2/neuplays a key role in the pathogenesis of gastric and oesophageal carcinomas and it's over expression has been documented in 6.8-34% of gastric carcinomas and 10-12.1% of oesophageal adenocarcinoma.Detecting the HER-2/neu status is a prerequisite for monoclonal antibody therapy.In this study, immunohistochemistry was used to detect HER-2/neu over-expression in gastric and oesophageal carcinomas. Objective: To associate HER-2/neu over-expression withage, sex, type and grade of gastric and esophageal carcinomas in upper gastrointestinal (UGI) endoscopic biopsies. Methods: HER-2/neu expression was investigated by immunohistochemistry on esophageal and gastric carcinomas ofUGI endoscopic biopsies received at our institution. Association between the expression of HER-2/neu andclinico-pathological parameters was statistically analysed. Results: The association was not statistically significant between age, sexand grade of the tumour with HER-2/neu overexpression. HER-2/ neu overexpression was seen in 14.2% of gastric adenocarcinomas, 20% of esophageal adenocarcinomas and 4% of esophageal squamous cell carcinomas. Predominantly, intestinal type (9.5%) of gastric carcinoma showed HER-2neu over-expression followed by diffuse type (2.3%) and mixed type (2.3%). Conclusion: In view of increasing trend of UGI tract malignancies and associated poor survival of advanced carcinomas, assessing HER-2/ neu over-expression in gastric and esophageal carcinomas is helpful to decide the utility of adjuvant targeted chemotherapy.

Breast cancer is the most common cancer in women globally. The factors that increase risk include: late age at ﬁrst birth, alcohol, radiation exposure, family history of breast cancer, and postmenopausal hormone therapy. Numerous drugs are being developed to treat breast cancer. Among them, Herceptin is used for the treatment of human epidermal growth factor receptor 2 (HER2)-positive cases and targets HER2 effectively and efficiently, but it is very expensive. Methylsulfonylmethane (MSM) is an organic sulfur-containing natural compound having no reported toxicity. We examined MSM in breast cancer cell lines and found it inhibited the proliferation of estrogen receptor-positive and HER2-positive breast cancer cells in a dose-dependent manner. It also suppressed the activation of STAT5b and expression of HER2 in breast cancer cells. We determined the STAT5b binding site (GAS element) in the HER2 gene. Detailed analysis showed that MSM decreased the ability of STAT5b to bind the promoter of the HER2 gene and a luciferase assay demonstrated reduced activity. We confirmed that MSM can effectively regulate STAT5b, and thereby decrease HER2 expression. Therefore, we recommend the use of MSM as an inhibitor for the management of HER2-positive breast cancers.

The National Institute of Standards and Technology (NIST) Standard Reference Materials 2373 is a set of genomic DNA samples prepared from five breast cancer cell lines with certified values for the ratio of the HER2 gene copy number to the copy numbers of reference genes determined by real-time quantitative PCR and digital PCR. Targeted-amplicon, whole-exome, and whole-genome sequencing measurements were used with the reference material to compare the performance of both the laboratory steps and the bioinformatic approaches of the different methods using a range of amplification ratios. Although good reproducibility was observed in each next-generation sequencing method, slightly different HER2 copy numbers associated with platform-specific biases were obtained. This study clearly demonstrates the value of Standard Reference Materials 2373 as reference material and as a calibrator for evaluating assay performance as well as for increasing confidence in reporting HER2 amplification for clinical applications.
