Hepatocellular carcinoma in the United States: Prognostic features, treatment outcome, and survival

Abstract: Despite the difference in the underlying etiology of HCC in this population compared with Asian patients, poor prognostic indicators are similar. In this large series of patients at a single Northeastern hospital, analysis of presenting clinical characteristics was found to offer useful prognostic information.

“…In the setting of cirrhosis and hepatocellular carcinoma, the clinical course of PVT may be dramatic: intractable ascites and repeated bouts of variceal haemorrhages may be followed by severe deterioration of liver function, hepatic encephalopathy and death (Witte et al 1985). Two large, retrospective studies have shown that presence of PVT may signi®cantly aect prognosis (Calvet et al 1990;Stuart et al 1996). However, the features associated with PVT in the setting of hepatocellular carcinoma have not been thoroughly studied.…”

“…4 However, this system is useful for identifying only end-stage patients, and provides no valid discrimination for less advanced patients. Several additional investigations to identify variables related to prognosis in France, 5,6 the United States, 7 Spain, 8,9 Italy, 10 Austria, 11 Hong Kong, 12 and Japan [13][14][15] have been published in the past 20 years.…”

“…[1][2][3][4][5][6] In addition, locoregional ablative therapies, such as percutaneous ethanol injection (PEI), percutaneous microwave coagulation therapies (PMCT), radiofrequency ablation (RFA), and surgery are restricted in patients with portal venous invasion (PVI), thus contributing to a poor prognosis. [7][8][9][10][11][12][13][14] Therefore, risk factors involved in PVI must be identified in order to determine therapeutic strategies and outcome. To clarify prognostic factors involved in PVI, we prospectively compared liver function, tumor markers, and previous treatment in 312 patients with HCC.…”
