Hepatobiliary Disorders in Celiac Disease: An Update

Abstract: This communication reviews recent literature and summarizes hepatobiliary abnormalities that may complicate the clinical course of celiac disease. A wide spectrum of hepatobiliary diseases has been described, including asymptomatic elevations of liver enzyme levels, nonspecific hepatitis, nonalcoholic fatty liver disease, and autoimmune and cholestatic liver disease. Moreover, in the majority of patients, liver enzyme levels will normalize on a gluten-free diet. In addition, celiac disease may be associated wi… Show more

“…On the other hand, asymptomatic elevation in liver enzymes is due to increased intestinal permeability resulting in translocation of gut bacteria, kupffer cell stimulation, and production of tumor necrosis factor-α (TNF-α), pro-inflammatory cytokine, and reactive oxygen species, resulting in nonalcoholic steatohepatitis (NASH). In addition to, CD share common immunogenic factors and immunopathogenesis with other hepatobiliary disorders such as autoimmune hepatitis, primary biliary cirrhosis and primary sclerosing Cholangitis that may contribute to asymptomatic elevation in liver enzymes (34).…”

“…Many forms of hepatic diseases can be seen in those affected with celiac disease, including non alcoholic fatty liver disease and Nonalcoholic steatohepatitis (NASH) [2]. Non-alcoholic fatty liver disease or NAFLD is said to the commonest cause of chronic liver disease in the developed world [3, 4].…”

“…Non-alcoholic fatty liver disease or NAFLD is said to the commonest cause of chronic liver disease in the developed world [3, 4]. Celiac disease is seen in 4 to 13% in those having NASH [2]. The exact mechanism of this association is not known but it is plausible that this may be due to the increased intestinal permeability along with malabsorption of choline, a lipotropic factor and vitamin B6 [2].…”

“…It is well recognized that the incidence of chronic disorders is escalating in India and fat consumption trends play a prominent role in obesity and related disorders, including type II diabetes, hypertension, cardiovascular diseases, nonalcoholic fatty liver disease, obstructive sleep apnea, gastroesophageal reflux disease, musculoskeletal disorders, a variety of cancers and a number of psychosocial concerns 1,2 . Functional role of fat as one of the major macro nutrient in the diet is manifold.…”
