Context: Hepatitis E virus (HEV) is well-known to be transmitted by the fecal-oral route. In addition, several studies in Europe and Asia had reported potential HEV transmission associated with blood transfusion, but this route is still uncertain.Objective: The aim of this study was to estimate the anti-HEV IgG seroprevalence among blood donors around the world using the Bayesian-based methods.Evidence Acquisition: A systematic review was performed using the PubMed/Medline, Scopus, Web of Science and Cochrane Library databases using the terms "prevalence", "hepatitis E" and "blood donors". Studies with a timeframe from inception to March 2016, in Roman characters, that had outcomes of interest such as prevalence of IgG antibodies were included. The estimation of anti-HEV IgG (presented as the event rate and 95% confidence intervals, CI) was performed using a Bayesian-based random effect model using the Comprehensive Meta-Analysis software, version 2. Pairwise meta-analyses and chi-square tests were used to assess significant differences between different sexes and ages. Results and Conclusions:Of the retrieved studies, 71 met the inclusion criteria, comprising a total of 113 316 blood donors. The prevalence rate was statistically significant in males and over 40 years old donors (P < 0.05). The overall estimation of the anti-HEV IgG prevalence was 0.058 (CI 95%: 0.049 -0.068). Subgrouping by region, the estimates were higher for Asia and the Middle East, respectively: 0.113 (CI 95%: 0.040 -0.278) and 0.112 (CI 95%: 0.081 -0.152). Europe, Africa, Oceania and America had an estimated prevalence between 0.014and 0.088. A relevant prevalence of anti-HEV IgG among blood donors was found worldwide, especially in Asia and the Middle East and in males and donors over 40 years. This review reinforces the view that HEV parenteral route infection needs to be further investigated and possibly screened for in transfused blood.

Introduction: Hepatitis E virus (HEV) infection is rarely reported in industrialized countries, but recent studies have revealed quite variable seroprevalence rates among European populations, including blood donors. In Serbia, very limited data about HEV seroprevalence are available. This study aimed to determine the prevalence of anti-HEV IgG antibodies and HEV RNA in the sera of volunteer blood donors in Serbia. Methodology: Serum samples from 200 volunteer blood donors were tested for the presence of anti-HEV IgG by enzyme-linked immunosorbent assay (ELISA) using ORF-2 HEV genotype 3 recombinant proteins as antigen, and for the presence of HEV RNA by nested reverse transcriptase polymerase chain reaction (RT-PCR). Results: In total, 15% of the volunteer blood donors were seropositive. The prevalence increased with age; 21.5%, 14.2%, and 5.4% HEV seroprevalence rates were found in individuals older than 51 years, between 31 and 50 years, and in those younger than 30 years of age, respectively. However, no HEV RNA was detected in any of the individuals analyzed. Conclusions: The prevalence of anti-HEV IgG among blood donors as representatives of the general population is quite high in Serbia compared to data from many European countries. One of the reasons for this could be the high prevalence of HEV among Serbian pigs and the traditional consumption of piglet meat in the country. The relatively high HEV seroprevalence found among Serbian blood donors indicates the need for further investigation.

ABS TRACTObjective: Hepatitis E virus (HEV) is responsible for fecal-oral transmitted human acute hepatitis in endemic regions. Especially in non-endemic region, zoonotic transmission of hepatitis E raises an important public health concern over food safety and zoonotic risk. Previous seroprevalence studies have reported average rates of HEV seroprevalence about 6% in Turkey. This study was performed to determine hepatitis E virus seropositivity in blood donors and relation of seropositivity between age groups and sexes. Materials and Methods: A total of 248 blood donors were randomly selected and included the study. Anti-HEV IgG seropositivities were determined by ELISA. Results: HEV seropositivity was found in 10 (4.0%) of the donors. There were no significant differences among age groups and sexes. Conclusion: According to the results, HEV seroprevalence in Erzurum was found lower than the average seroprevalence of Turkey. The point that should not be ignored that, populations dealing with stockbreeding are at risk for zoonotic transmission. An extended seroepidemiological study on these populations with a detailed questionnaire may contribute to the identification of zoonotic risk factors. (
