Hepatitis C virus and its cutaneous manifestations: treatment in the direct‐acting antiviral era

Abstract: New all-oral direct-acting antivirals (DAA) have changed the hepatitis C virus (HCV) treatment landscape. Given that dermatologists frequently encounter HCV-infected patients, knowledge of the current treatment options and their utility in treating HCV-associated dermatologic disorders is important. In addition to highlighting the new treatment options, we review four classically HCV-associated dermatologic disorders - mixed cryoglobulinaemia (MC), lichen planus (LP), porphyria cutanea tarda (PCT) and necrolyt… Show more

“…Direct-acting antiviral (DAA) therapy achieves SVR rates over 90% and is currently recommended for patients with chronic HCV, based on the degree of liver fibrosis, HCV genotype, resistance variants, previous treatment failure and comorbidities [77,78]. The advantages of DAA over IFN-α and ribavirin therapy are an increased response rate and reduced length of therapy, i.e.…”

“…C hronic viral infections are an important worldwide health concern. Globally, approximately 240 million people are chronically infected with hepatitis B virus (HBV) 1 , more than 170 million are infected with hepatitis C virus (HCV) 2,3 , and 75 million are infected with human immunodeficiency virus (HIV) 4 . These viruses can lead to persistent infection and dysfunction of adaptive immunity 2,5 .…”

“…This second hypothesis was further supported after the publication of case reports associating the involution of OLP and other EHMs 23,24 to the SVR with the use of AAD, [25][26][27] including re-establishment of immune tolerance in the patients. [28][29][30] These findings make us to suggest that the ideal treatment for cases of HCV-associated OLP should be coupled with the establishment of SVR.…”
