Hepatitis C virus: A rare manifestation - Remitting relapsing central and peripheral demyelination

Abstract: The most frequent neurologic manifestations of hepatitis C virus infection include peripheral neuropathy axonal type and central nervous system (CNS) vasculitis. Affected patients usually have cryoglobulinemia and other signs of vasculitis. Demyelinating lesions, both central and peripheral are rarely described. We present a case of simultaneous peripheral nervous system and CNS demyelination that comes in relapsing episodes, with negative cryoglobulins.

“…Most peripheral neuropathy in patients with HCV infection is the axonal type of symmetrical sensorimotor neuropathy or multifocal mononeuropathy. Peripheral demyelinating neuropathy has also been rarely described, most often in cryoglobulin negative patients (13,14). The link between HCV and demyelinating neuropathies could be related to a virus-triggered immune-mediated process (15).…”

“…In the relevant literature, symmetric sensorimotor PN of mostly the axonal type and mononeuropathy multiplex are reported in association with hepatitis C infection (22). However, in patients lacking cryoglobulin, demyelinating PN dominate (23,24).…”

“…In both cases, patients improved with steroid treatment. Mestre et al [88] and Bezerra et al [89] reported cases in which patients developed both peripheral and CNS demyelination. Moreover, the patient reported by Bezerra et al had relapsingremitting demyelination involving the brain and spinal cord, following a course like multiple sclerosis (MS).…”
