Hepatitis B and C virus coinfection in The TREAT Asia HIV Observational Database

Abstract: The impact of hepatitis coinfection on immunological and virological responses to antiretroviral therapy and HIV disease progression among this Asian cohort are similar to that seen in Western countries. The longer-term impact of hepatitis coinfection on both HIV disease and liver disease morbidity and mortality needs to be monitored.

“…ALT activity was significantly higher among HIV/HBV co-infected participants compared to HIV monoinfected ones. This is in agreement with findings resulting from other investigations in Cameroon and worldwide in which high level of ALT was reported (Zoufaly et al, 2012;Zhou et al, 2007). The ALT is found in serum and in various bodily tissues, but high level in the serum is most commonly associated with the liver damage (e.g.…”

“…It is generally recognized among injecting drug users in HBV highly endemic areas, such as South East Asia [69]. In high-income countries, new HCV epidemics are reported among young white adults living in rural areas in the United States abusing opioids intravenously and among promiscuous homosexual men in Western Europe, North America, and Australia.…”

“…The patient should be treated with pegylated interferon alpha and ribavirin, as indicated for chronic hepatitis C. [56] Sustained virological responses are similar to reponses in those who are monoinfected. [57][58][59] In individuals with chronic hepatitis C genotype 2 and 3, if the chronic hepatitis B requires treatment, then pegylated interferon alpha could be continued for 1 year or NUC therapy considered. There is a potential risk of HBV reactivation during treatment or after HCV clearance, which should be treated with NUCs.…”
